Objective:AIDS is known as "super cancer" and "century killer,which still remains a major threat to human health during this century.HAART(Highly Active Anti-Retrovirus Therapy)has dramatically reduced the mortality of AIDS.However,many problems still exist in the HAART therapy,such as side effects,need long-term medication,and the prone to drug resistance is still difficult to find solution.The effect of Chinese medicine has an irreplaceable role to other drugs in improving HIV/AIDS patients with clinical signs and symptoms,improve quality of life significantly,but it is still a lack of recognition of its evaluation of the efficacy of consensus.Clinical observations indicate that the Ai Ke Qing capsules,the main principles prescription from kidney detoxification can significantly increase the weight of the HIV infection,and improve the quality of life.Therefore,to explain the mechanism of kidney detoxification method on AIDS interventions in the role of the mechanism,will have a great significance to reflect the traditional Chinese medicine treatment of AIDS scientific evaluation of the efficacy,the traditional Chinese medicine treatment of AIDS science,as well as large-scale promotion of the application.Monkey AIDS model is the recognized "gold standard" for AIDS researching,and extensive be used in AIDS pathogenesis and anti-AIDS medicine and vaccines;We have developed the model and work on the research for many years.This research program is mainly focus on the immunology,virology,pathology,kidney detoxification and formulas intervention in the monkey AIDS model.Methods:To use 15 domestic rhesus monkeys with experimental requirements and has intravenous infection of SIVmac239,randomly divided into three groups:low dose group(normal clincail trails dosage),high dose group(3 times of normal clincail trails dosage),and model control group,each group has 5 monkeys.To start the medicine treatment after 77 days of Monkey infection and continuing administration for eight weeks,and gave saline orally to model control group during the same period.Collected once of peripheral blood every 4 weeks,and collected once of superficial lymph nodes every 8 weeks.First,the experiment use quantitative PCR to detect peripheral blood viral load,flow cytometry to detect T cell subsets,pure subsets,and memory subsets,and enzyme-linked immunosorbent to detect the CD56 of NK cell surface expression and IL-21 levels in peripheral blood.Secondly,use radioimmunoassay method to detected the serum T3(three iodine Thyronine),T4(thyroxine),COR(cortisol)and beta2-MG(serum beta 2-microglobulin),and the result of indicators are closely related with spleed and kidney deficiency syndrome.Last step to work on the pathological ananlysis for the superficial lymph nodes,and HE staining of monkeys in each group,and compare the dynamic changes of the pathological features of the 15 Monkeys by the defected results of peripheral blood and immune superficial lymph nodes.Results:1.The experiment found the trend of CD4 + C28 + and CD8 + C28 + in the three groups is similar,but the trend of CD4 + CD28-and CD8 + CD28-in the three groups is opposite:it decreased in the control group,while the treatment group increased,especially in the low-dose group increased significantly.Large numbers of CD8 + CD28-cells increased in the low dose group,which is may cause by produce immune tolerance,thereby inhibiting the CD4 activation,proliferation,Therefore,during the declined of CD4,no significant increase in viral load 'nor reduce body weight,and also did not appreared death monkeys.2.During the experiment period,the three groups of memory cells of CD4+ CD45RA-and CD8 + CD45RA-were decreased,which in line with AIDS regular progression.The trend of Pure cell CD4 + CD45RA + and CD8 + CD45RA + in the three groups are different:CD4 and CD8 cells of innocence decreased in the control group;CD8 cells of innocence was significantly higher in the low-dose group;CD4 and CD8 cells of innocence are both increased in the high dose group.We speculate that the rise of the pure cells may be an important factor in patients taking kidney detoxification recipe and improve their survival quality.If kidney detoxification recipe combine with HAART treatment,and make patients pure T lymphocytes increased appear ahead of time,then it will be shortered the immune reconstitution time for the patients,and avoid immune reconstitution before infection,which is an advantage for the patients.3.Compare with the control group,it is not observed significantly lower viral load in kidney detoxification recipe,which may releated to the highest viral load of no.541 monkey in the control group died during the experiment,and the no.543 monkey also in the contro group was controlled viral load to undetectable level,thus the resulting in viral load volume number of the control group was lower than that of the treatment group.Further experiment found that IL-21 content is related to the Copy Log strong correlation with viral load.The no.543 monkey with significantly high content of IL-21 can control the viral load well;this is a significant guidance for vaccine developing and AIDS prevention,and the mechanism worthy for further study.The high-dose IL-21 content is a slight increase with time,so we speculate the kidney detoxification recipe may able to effect the increasing of IL-21 content.4.The experiment found:Prior to therapy as 2 and half months after infection,some of the experimental monkeys T4 value lower than the normal range,and began to appear deficiency syndrome;In the distherapy of 8 weeks,as 6 months after infection,the three groups all appeared T3,T4,COR different degrees of decline,and the COR decline with statistically significant.However,the COR value in the three groups of monkeys are all in the normal range,which meant the SIV monkey models have kidney deficiency syndrome trend after they were infected six months,which prompted need to strengthen the efforts of the kidney when enter this stage.For these indicators,the treatment group and the control group did not find significant difference.5.Monkey lymph node pathology:as the duration of the extension,the lymph node structure of the monkeys in the SIV control group deteriorated gradually,this is in line with the regular natural pathological changes of the SIV-infected monkeys.In the treatment group,the low-dose and high dose group regardless of the lymph node structure remained stable during treatment or after stop therapy,and even“turn to better”.It speculated the traditional Chinese medicine has therapeutic efficiency of both high and low dose groups of SIV monkey model,and there was a dose-effect relationship.Conclusion:Although the kidney detoxification method and formulas do not show significant inhibition to SIV virus,it increased the number of pure T cells,and certainly also increased the serum IL-21 content,which is exactly a valuable point.In addition,in terms of pathology,whether it is during treatment or after stopping treatment,the group lymph node structure remained stable,or even“turn to better”.It prompts kidney detoxification methods has a therapeutic effect to SIV monkey models.We speculated these factors may be the reason of improving the patients quality of life with clinical.
