Research On Coronary Heart Disease Costs Based On Data Mining Technology

[Objective]To analyse the status of medical service data quality of 39 hospitals over two levels in Beijing,propose the data cleaning scheme,and construct the database of disease cost.To establish a analysis framework based on patients,hospitals,diseases,surgery and other dimensions of disease costs.Based on the analytical framework,the status and characteristics of the cost of patients with coronary heart disease are studied.Data mining technology is used to find out whether there are different rules of diagnosis and treatment in the process of diagnosis and treatment of coronary heart disease,the status of cost under different laws and the main factors that affect the cost of patients,so as to provide a new way of thinking for the management and control of disease costs.[Methods]Through literature review,summarize the data cleaning process,the methods of data problem detection and cleaning methods.To study the research progress of data mining technology such as decision tree and association rule,and slelect the suitable data mining model for medical expense.To describe the distribution,concentration and discrete trend of the cost of coronary heart disease patients.The association rule model was constructed to excavate the combination rule of coronary heart disease complications and complications and the rules of drug and hygienic materials.The decision tree grouping model is used to explore the main factors that affect the cost of patients.[Results]In view of the existing problems in the data,we should detect,merge,delete,mark,replace and so on,and construct the relational database of the cost of diseases.And form data processing flow including data detection,data cleaning,data validation,clean data backflow,and database construction.Based on the cost of disease framework,combined with the purpose of the study,the cost of coronary heart disease in 39 hospitals in Beijing from 2014 to 2016 was statistically described.(1)The age of the patients with coronary heart disease had a partial distribution,and the patients were mainly concentrated in the age group of 55 years old-85 years old.The proportion of the outpatients was balanced and the sex ratio of inpatients was 6:4.(2)Patients were mainly chronic ischemic heart disease,and the proportion of chronic ischemic heart disease in outpatients was 84.97%,the proportion of inpatients was 62.61%.(3)The average expense of outpatients was 662.36 yuan,with a median of 509.17 yuan,the average expenses of inpatients were 25.8 thousand yuan,the median was 12.3 thousand yuan,and the dispersion trend of expenses was significant.(4)The cost of acute myocardial infarction is the highest,and there is also a significant difference in the cost of the same disease in different hospitals.(5)Surgical patients were mainly distributed in hospitalized patients,and the average expenses were higher than those of non operative patients.(6)Outpatient expenses are mainly drug expenses,accounting for more than 80%,and the expense of inpatients is mainly health materials,accounting for 51.86%.First,a association rule model of patient diagnosis was constructed to obtain multiple diagnostic rules,such as simple acute myocardial infarction,simple chronic ischemic angina,chronic ischemic angina with cerebrovascular disease,chronic ischemic heart disease with diabetes and metabolic disorder.There were significant differences in the cost of patients with different diagnostic rules,among which patients with simple acute myocardial infarction had the highest cost,and in patients with associated diseases,the average cost of hospitalization for chronic ischemic heart disease accompanied by diabetes and metabolic disorders was the highest.According to the different diagnostic rules of patients,construction of medicines and medical materials Association rules.The results showed that the usage rules of drugs and consumables in different diagnostic rules were common,but they also had their own characteristics.In patients without accompanying diseases,the use of drugs and health materials is more regular,and the effect of rules on expenses is more significant.The use of drugs and health materials is more dispersed in patients with accompanying diseases,and those outside the rules may have greater impact on the cost.Based on the use rules of drugs and health materials,combining the patient's personal basic information and hospital,the decision tree group model is constructed to explore the main reasons for the cost difference.The results showed that drug usage rules,consumables usage rules,visiting hospitals and hospitalization days had different effects on expenses.In patients with no accompanying disease,the rules of medicine and health materials have a greater impact on the cost,while the days of hospitalization,the hospital and the operation are more likely to affect the patient's cost when the disease is associated with the disease.[Conclusion]The trend of cost discretization in patients with coronary heart disease was significant.The cost difference of patients may be caused by reasonable reasons,but there may also be unreasonable factors.We need to dig deeper into the causes of the cost difference.The average hospitalization expenses of patients with coronary heart disease and those with concomitant diseases are significantly different.Different diseases will also lead to different hospitalization expenses.The study of the association rules and costs of the diagnosis will help to further understand the distribution of the disease,provide a reference for the reform of medical insurance payment,the formulation of the payment standards,and provide the basis for the hospital control.There are rules for the use of drugs and health materials,which affect the cost of patients.The search for the use of drugs and sanitary materials can provide ideas for the standard of clinical diagnosis and treatment,provide reference for the production,supply and price policy of drugs and consumables,monitor the diagnosis and treatment of the hospital,and understand the effect and influence of the policy.Hospitalization days,drug use rules,rules for the use of health materials,hospitals and other factors affect the hospitalization expenses of patients.It is helpful to find the key points of the cost control,provide the basis for the management and control of the cost of the disease,and provide the thought for the fine management of the hospital.
