| Stroke,also known as the“cerebrovascular accident”,modern medicine is divided into two types of hemorrhagic and infarction,with high incidence rate and high disability rate,high recurrence rate and high mortality rate of the “four high” characteristics,in acupuncture department and neurology ward,rehabilitation department is a clinical common disease and frequently-occurring disease.With the change of the pace of life and habits,individual psychology and work pressure increase,the incidence trend is becoming more and more young;And,according to reports,after the success of the first aid,stroke patients within 1 week,about 73%-86% of the patients with partial body limb disorders,with 71%-71% are given priority to with operation difficulty,47% performance cannot sit alone,resulting in a serious decline in quality of life in patients.After spinal shock stage,patients with excessive increase muscle tension,seriously affected the recovery of limb function in patients;especially high spasticity.Currently,the treatment of spasticity after stroke is mainly nonpharmacological and non-surgical,more in rehabilitation,acupuncture and other treatments.Rehabilitation techniques are based on neurophysiology and developmental principles,promoting the recovery of central nervous system by repeated exercise and training,widely used by the clinic.The effectiveness of acupuncture in the treatment of spasticity after stroke had been recognized both at home and abroad.The operation methods were generally divided into:electro-acupuncture,fire needle,body acupuncture,scalp acupuncture,acupuncture combined with moxibustion and abdominal acupuncture,etc.Among them,there are many clinical reports about electro-acupuncture.Secondly the fire needle,fire needle therapy was a special needle with fire,heated needle body,imposing a certain technique,quickly penetrating into the corresponding acupoints or parts of the human body,a unique acupuncture treatment to treat and prevent diseases.But electro-acupuncture,fire needle therapy after stroke of spasticity are still in the stage of academic debate.Each theory is not the same;At present,there are few comparative studies between different external therapies and the same external therapies on the different intervals.Based on this,on the basis of rehabilitation treatment,the research discussed efficacy study between fire needle and electro-acupuncture and among different intervals of electro-acupuncture for the same disease(spasticity after stroke).ObjectiveThrough detailed grouping and curing treatment times and curing time,observe different treatments and different intervals of the clinical curative effect of treatment of spasticity after stroke,namely,the clinical curative effect of fire needle,rehabilitation with different intervals of electroacupuncture,under the premise of maximum efficacy,coming to the conclusion of the reasonable treatment of the disease,to reduce patient's pain and social economic pressure,improving the patients quality of life,to promote the standardization of acupuncture and moxibustion therapy and standardization process.Methods To conform to the clinical research into the cases,using the random number table method for random grouping,namely:the rehabilitation group,the fire needle group,daily electro-acupuncture group,every other day electro-acupuncture group and every three days electro-acupuncture group;Rehabilitation treatment as the basis of each group.By Fugl-Meyer Assessment(FMA),Modified Barthel Index(MBI),clinic spasticity index(CSI)method evaluation and Surface EMG,respectively before treatment,unified treatment of 1 month and unified treatment of 30 times for the node,the patients were assessed,the results of data for the statistical analysis.Results1.Through clinical observation,on the basis of a unified treatment time(1 month)and rehabilitation therapy,(1)in the aspects of FMA score,MBI score and CSI score,comparison of efficacy before and after treatment,the treatments were better than before treatment;(2)among the groups of FMA score,MBI score and CSI score,as follows:(1)Due to differences in treatment interval,the daily electro-acupuncture group had the highest number of acupuncture treatment.The curative effect was better than that of every other day electro-acupuncture group and every three days electro-acupuncture group,P<0.05.The daily electro-acupuncture group had the best effect;The curative effect of every other day electro-acupuncture group was better than that of every three days electro-acupuncture(P<0.05).(2)Based on the same interval time,the fire needle group was better than every three days electro-acupuncture group,P<0.05.Based on different intervals,the effect of the fire needle group and every other day electro-acupuncture group were basically equal,P>0.05;Effect of the fire needle group was less than daily electro-acupuncture group,P<0.05.(3)Rehabilitation therapy was used as the basic treatment for each group,and the number of rehabilitation treatment in each group was equal.The rehabilitation group was treated with rehabilitation alone.The effect of rehabilitation group was less than fire needle group,daily electro-acupuncture group,every other day electro-acupuncture group and every three days electro-acupuncture group,P<0.05.2.On the basis of the number of uniform treatment(30 times)and rehabilitation therapy,(1)in terms of FMA score,MBI score,CSI score and surface electromyography,comparison of efficacy before and after treatment,the treatment was better than before treatment;(2)the FMA score,MBI score,CSI score and surface electromyography were used between the groups,as follows:(1)Based on the same number of treatments and the same duration,the fire needle group was better than every three days electro-acupuncture group,P<0.05;Depending on the same number of treatment times and the different duration,Effect of fire needle group was superior to those in the every other day electro-acupuncture group and daily electro-acupuncture group,P<0.05.(2)External acupuncture therapy based on the same treatment times with different durations,the daily electro-acupuncture group had the same curative effect as the every other day electro-acupuncture group,P>0.05.But both were better than the three days electro-acupuncture group,P<0.05.(3)Rehabilitation therapy was used as the basic treatment for each group,and the number and time of rehabilitation treatment in each group were equal.The rehabilitation group was treated with rehabilitation alone.The effect of rehabilitation group was less than fire needle group,daily electro-acupuncture group,every other day electro-acupuncture group and every three days electro-acupuncture group,P<0.05.Conclusion1.The simple rehabilitation therapy little pain,can improve the state of spasticity after a stroke,should be used as the based therapy for the treatment of spasticity after stroke.2.In a fixed time(1 month)should be given the high frequency of acupuncture and moxibustion treatment,in a short period of time to enhance the clinical curative effect,so that,let patients establish the confidence in the rehabilitation of the disease as soon as possible.3.In a fixed number of times(30 times)after treatment,the clinical curative effect of fire needle was better than that of rehabilitation group,daily electro-acupuncture group,every other day electro-acupuncture group and every three days electro-acupuncture group,and daily electro-acupuncture group and every other day electro-acupuncture group were no significant difference in efficacy,and both were better than the every three days electro-acupuncture group.4.Combined with the patient's condition and the characteristics of the disease,for long-term treatment over the course of 3 months,based on enhancing effect,selection of fire needle and every other day electro-acupuncture is appropriate,and can relieve the pain of patients and social and family economic burden;For the treatment process is shorter than 1 months of acute stage or treatment of acute disease,on the basis of improving the curative effect,the choice of electro-acupuncture treatment program is more reasonable,can quickly enhance the confidence of patients with rehabilitation.Innovation point1.The research set up different external treatment and different interval,on the basis of the therapeutic effect,to speculate the best and most suitable electro-acupuncture treatment method and interval.To reduce patient pain,maximum clinical benefit.Under the influence of excluding non intervention measures,extended observation period,closer to clinical practice,the nodes of the duration of treatment(1 month)and the number of times of unified treatment(30 times),conducting evaluation,more objectively reflected the different intervals of therapeutic effects.2.For post stroke spasticity,the current efficacy evaluation based on the scale,lack of objective index description,to more accurately reflect the efficacy of the objective and intuitive,on the basis of the scale evaluation,add surface EMG evaluation index,to enhance the reliability of the results evaluation. |
