Research On The Influence Of Internet Hospital Health Education On Self-management Of Community Hypertension Patients

Research Purpose:Compare the Internet hospital health education and community health education on hypertension patients in community of the self-management behavior,self-efficacy behavior,hypertension knowledge,blood pressure and other aspects.Explore an effective way of health education for self-management of community hypertension patients.Research Methods:This study is an experimental study.Through telephone contact,patients with primary essential hypertension who are registered in xindong community hospital and meet the criteria of Inclusion and exclusion were selected as subjects.According to the random number table method,130 subjects were randomly divided into the control group and the experimental group.The control group was given lectures on hypertension health education in the community,and the activities were 20-minute health lectures and 20-minute nurse-patient communication.The objectives were followed up by telephone every two weeks.If the subjects have hypertension-related problems,they can contact by telephone or receive free guidance and assistance at the hospital.The experimental group gave lectures on hypertension health education knowledge on the Internet hospital App.First,videos were made and uploaded to the Internet hospital App.The video includes the courseware's PPT of the current activity in the control group,and the video(portrait + voice)of the propagandist is added,which is placed in the lower right corner of the video for the patients to watch at any time and repeatedly.Internet hospital doctors and nurses give online medical treatment every day,and patients can be followed up online at any time(no less than once every 2 weeks).If the subjects have hypertension-related problems,the Internet hospital is available for free consultation at any time.Combining the adult learning theory and referring to the material like "guidelines for the prevention and treatment of hypertension in China(revised edition in 2018)",6 health education lectures were designed,including the self-introduction and basic knowledge of hypertension,the method and significance of monitoring blood pressure correctly,medication management,diet guidance,exercise guidance,reasonable lifestyle and the summary of activity.Both groups conducted activities once a month for 6 times in total,and the intervention period was 6 months.The evaluation index includes main evaluation index and secondary evaluation index.Main evaluation index including group score indicators(self-management behavior score,self-efficacy scores,hypertension related knowledge score)and community health indicators(self-management behavior improvement rate,self-efficacy behavior improvement rate,hypertension related knowledge improvement rate,medication compliance improvement rate,blood pressure improvement rate).Secondary evaluation indexes including blood pressure,the number of hospitalized patients with cardiovascular diseases within half a year,activity attrition rate,experimental group relearning rate and two groups counseling rate.To evaluate and compare the effect of Internet hospital health education and community health education on the self-management of hypertension patients by comparing within and between groups.Research Results:A total of 120 cases of hypertension patients were collected in this study,including 60 cases in the control group and 60 cases in the experimental group.The research results are as follows:1.Baseline comparisonBefore the intervention,there was no statistically significant difference between the two groups in the general information,self-management behavior score,self-efficacy behavior score,hypertension related knowledge score,the number of good self-management,the number of good self-efficacy,the number of good hypertension knowledge,the number of good medication compliance,the number of good blood pressure control,blood pressure,the number of hospitalized patients with cardiovascular diseases within half a year(P > 0.05).The baseline was consistent and comparable.2.Main evaluation indexes comparison2.1 Group score indicators comparisonThe self-management behavior score,self-efficacy scores,hypertension related knowledge score of the two groups were compared and analyzed within the group.The self-management behavior score,self-efficacy scores,hypertension related knowledge score of two group were higher than that before the intervention,and the difference was statistically significant(P < 0.05).The comparison between groups showed that the score of the experimental group was higher than that of the control group,and the difference was statistically significant(P < 0.05).Both Internet hospital health education and community health education can effectively improve hypertensionpatients' self-management behavior,self-efficacy behavior,knowledge level related to hypertension,and the intervention effect of Internet hospital health education is better.2.2 Community health indicators comparisonComparison and analysis within the group showed that self-management behavior improvement rate,self-efficacy behavior improvement rate,hypertension related knowledge improvement rate,medication compliance improvement rate,blood pressure improvement rate(including diastolic blood pressure improvement rate and systolic blood pressure improvement rate)are all improved in the experimental group,with statistically significant differences(P < 0.05).The self-management behavior improvement rate,medication compliance improvement rate,and systolic blood pressure improvement rate in the control group were improved,with statistically significant differences(P < 0.05).The blood pressure improvement rate in the experimental group was better improved,and the difference was statistically significant(P < 0.05).Internet hospitals can effectively improve the self-management improvement rate,self-efficacy improvement rate,hypertension knowledge improvement rate,medication compliance improvement rate,blood pressure improvement rate of community hypertension patients.Community health education has a good effect on improving self-management behavior improvement rate,medication compliance improvement rate,and systolic blood pressure improvement rate.In the aspect of blood pressure improvement rate,the effect of Internet hospital health education is better than that of community health education.3.Secondary evaluation indexes comparison3.1 Blood pressure comparisonThe comparison and analysis of the blood pressure of the two groups showed that the systolic and diastolic blood pressure of the control group and the experimental group were lower than those before the intervention,and the difference was statistically significant(P < 0.01).The comparison between groups showed that the systolic and diastolic blood pressures of the experimental group were lower than those of the control group,and the difference was statistically significant(P < 0.05).Two groups of patients with high blood pressure blood pressure difference before and after the intervention,there is a decrease in the systolic blood pressure difference between two group were(control group fell 5.62±5.23 mm Hg,experimental group fell 7.93±7.34 mm Hg),the difference was statistically significant(P < 0.05),diastolic blood pressure differencebetween two group were decreased(control group fell 2.30±5.61 mm Hg,experimental group fell6.15±2.92 mm Hg),the difference was statistically significant(P < 0.05).Compared with community health education,Internet hospital health education can effectively reduce systolic and diastolic blood pressure in patients with hypertension,and Internet hospital health education has better effect on blood pressure control.3.2 The number of hospitalized patients with cardiovascular diseases within half a year comparisonComparison and analysis within the group showed that the hospitalization rate of hypertensive patients in the control group and the experimental group were both decreased,but the difference was not statistically significant(P > 0.05).There was no statistically significant difference between the two groups(P > 0.05).3.3 Activity attrition rate comparison between the two groupsFive subjects were missing in the experimental group and the control group,and the activity attrition rate of both groups was 7.69%.3.4 Comparison of the experimental group relearning rate and two groups counseling rateIn the experimental group,59.17% of the subjects watched learning on the same day,40.83%watched learning in the future,and 12.33% watched learning for two or more times.In half a year,the number of telephone consultations in the control group was 98,and the number of Internet consultations in the experimental group was 126.Research Conclusion:Internet hospital health education can effectively ameliorate community hypertension patients' blood pressure,enhance their self-management behavior and self-efficacy behavior,improve hypertension knowledge and medication compliance,improve self-management behavior improvement rate,self-efficacy behavior improvement rate,hypertension related knowledge improvement rate,medication compliance improvement rate,blood pressure improvement rate in community hypertension patients.The effect is better than the traditional community hypertension health education.
