Objective:The subject of clinical research methods,the use of acupoint catgut embedding with ear pressure treatment of liver qi stagnation of premenstrual syndrome,the establishment of commonly used acupuncture group,oral proprietary Chinese medicine as a control group.Total score of TCM syndrome,(E2),progesterone(P)and SCL-90 self-rating scale were used to explore the efficacy of premenstrual syndrome with depression of liver qi.Methods:The clinical study cases were from the Sun Medicine Clinic and Catholic New Store Gengxin Hospital.From January 2016 to January 2017,90 patients with premenstrual syndrome were collected.Patients were randomly assigned into the catgut embedding with ear pressure group,the acupuncture group and the traditional Chinese medicine group,with 30 cases in each group.Western diagnostic criteria refer to the American Society of Obstetrics and Gynecology(ACOG,American College of Obstetricians and Gynecologists)published in 2000 on the diagnosis of premenstrual syndrome,TCM syndrome differentiation with reference to the Chinese People's Republic of Chinese medicine industry standard "TCM diagnosis and treatment criteria"(Nanjing University Press,1994)developed by the premenstrual syndrome,"liver qi stagnation type"diagnostic criteria.Acupoints selection for acupoint catgut embedding group:Taichong,Sanyinjiao,Qimen,Ganshu,auricular acupressure acupoints:liver,kidney,endocrine,Shenmen,genitalia.One time treatment of acupoint catgut embedding were given 14 days before the menstrual cycleand auricular acupressure pressure 14 days before the menstruation cycle too,two to three days for a pair of ears alternately.Total treatment of three menstrual cycles.Premenstrual syndrome syndrome of traditional Chinese medicine:According to the "clinical diagnosis and treatment of traditional Chinese medicine standard"(State Administration of Traditional Chinese Medicine published on June 18,1994)and "Chinese medicine treatment of premenstrual tension syndrome clinical research guidelines" And SCL-90 self-rating scale evaluation.Endocrine hormone determination of estradiol(E2),progesterone(P).ResultsBefor the treatment,we compared all the patients' age,course of disease,the scores of SCL-90 self-rating scale,estradiol(E2),estradiol(E2),estradiol(E2),total score of TCM syndrome.The results showed that there was no significant difference between the three groups before treatment(P>0.05),indicating that the clinical baseline data between the three groups were comparable.? Comparison of total score of TCM syndrome in premenstrual syndromeAfter two menstrual cycles,the total score of TCM syndrome of the the catgut embedding with ear pressure group and the traditional Chinese medicine group was lower than that before the treatment,with statistical significance(P<0.05)..The score of the conventional acupuncture group was not obviously improved(P>0.05),and there was no significant difference between the three groups(P>0.05).After three menstrual cycles,the total score of TCM syndrome of the premenstrual syndrome in the catgut embedding with ear pressure group was significantly different as compared with that before treatment(P<0.01).The total score of TCM syndrome of the two control groups also had different degree drop than that before treatment(P<0.05).Comparison among the thress groups showed that the total score of TCM syndrome of the premenstrual syndrome in the catgut embedding with ear pressure group were significantly lower than those in the conventional acupuncture group and the traditional Chinese medicine group(P<0.05).Tip:in the improvement of TCM syndrome of patients with liver qi stagnation type of premenstrual syndrome,all of the three kinds of therapies didn' t get a good clinical efficacy after a menstrual cycle;after two menstrual cycles treatment,catgut embedding with ear pressure and traditional Chinese medicine treatment can improve the TCM symptoms of patients with premenstrual syndrome;after three menstrual cycles,the clinical efficacy of the three groups was more obvious.In contrast to the different therapies,it was clear that the catgut embedding with the ear pressure therapy had much better effect of improving TCM symptoms of liver qi stagnation type of premenstrual syndrome as compared to conventional acupuncture and oral Chinese medicine Danzhi Xiaoyao pill.?Comparison of scores of all items of TCM syndrome in premenstrual syndromeOn the improvement of liver qi stagnation of premenstrual syndrome patients with TCM syndromes,such as headache,irritability,breast pain,swelling,emotional depression,etc.,after a menstrual cycle of treatment,three different therapies didn't get good clinical efficacy.After two menstrual cycles,catgut embedding combined with ear pressure therapy had a certain effect on relieving headache,irritability and depression in patients with liver qi stagnation type of premenstrual syndrome.Routine acupuncture could also improve headache and depression symptoms;traditional Chinese medicine was effective in improving headache,irritability and breast distending pain.After three menstrual cycles treatment,catgut embedding with ear pressure therapy had a obvious effect in improving the TCM symptoms of patients with liver qi stagnation type of premenstrual syndrome,although conventional acupuncture and Chinese medicine also have varying degrees of efficacy,catgut embedding with ear pressure had a more obvious synergistic effect,the overall effect was better than a single conventional acupuncture and traditional Chinese medicine treatment,and it performed significantly better in a long time.?SCL-90 self-rating scale score comparisonAfter the two menstrual cycles,the SCL-90 self-rating scale score of the catgut embedding with ear pressure group was decreased(P<0.05).Although the SCL-90 self-rating scale score was reduced in both of the conventional acupuncture group and the traditional Chinese medicine group,the change had no statistical significance(P>0.05).Compared with the traditional Chinese medicine group,the SCL-90 self-rating scale score of the catgut embedding combined with the ear pressure group was statistically significant(P<0.05).After three menstrual cycles,the SCL-90 self-rating scale score of the catgut embedding with ear pressure group was significantly different from that before treatment(P<0.01).SCL-90 self-rating scale score in the conventional acupuncture group and the traditional Chinese medicine group also had some extend of improvement(P<0.05).The SCL-90 self-rating scale score of catgut embedding combined with ear pressure group was compared with that of conventional acupuncture group and traditional Chinese medicine group,the difference was statistically significant(P<0.05).Note:SCL-90 self-rating scale could reflect a wide range of psychiatric symptoms,involving emotions,thinking,awareness,behavior and other aspects,from the results we knew that a single menstrual cycle treatment didn't have clinical efficacy;after two menstrual cycles of catgut embedding with ear pressure treatment,the psychological symptoms during women's menstrual period could be improved obviously,and the effect was better than traditional Chinese medicine treatment;after three menstrual cycles,all of the three therapies had a differing effect in reducing the SCL-90 self-rating scale score of patients with liver qi stagnation type of premenstrual syndrome.On the contrast of efficacy,the catgut embedding combined with ear pressure therapy was significantly better than conventional acupuncture and traditional Chinese medicine treatment.? Comparison of factor scores of SCL-90 self-rating scaleAfter a menstrual cycle treatment,for the patients with liver qi stagnation type of premenstrual syndrome,all of the three therapies didn't have obvious effect for the improvement of somatization symptoms,interpersonal sensitivity and psychiatric symptoms such as depression,anxiety,hostility,obsessive-compulsive symptoms,paranoia and fear x.After two menstrual cycles,catgut embedding combined with auricular acupoint therapy could improve the somatization symptoms,depression and anxiety of patients with premenstrual syndrome.The treatment of Chinese medicine had only a certain effect on depressive symptoms.After three menstrual cycles,catgut embedding combined with ear pressure therapy could relieve the somatization symptoms and psychiatric symptoms such as obsessive-compulsive symptoms,depression,anxiety and paranoia in patients with premenstrual syndrome,and it was superior to conventional acupuncture group and traditional Chinese medicine group in curing depression and anxiety.However,the clinical effect of three groups were not significant for improving the interpersonal sensitivity and hostility.? comparision of estradiol(E2)values among the three groups after treatmentIntra-group comparison:no matter in which menstrual cycle treatment,no statistical significance was found while compring the estradiol(E2)value of the catgut embedding combined with ear pressure group,the conventional acupuncture group and the traditional Chinese medicine group with those before treatment(P>0.05).Intergroup comparison:no matter in which menstrual cycle treatment,no statistical significance was found as we compared the estradiol(E2)value between the three groups(P>0.05).It was suggested that catgut embedding combined with auricular acupressure,conventional acupuncture and traditional Chinese medicine treatment had no obvious effect on improving estradiol(E2)level in patients with liver qi stagnation type of premenstrual syndrome.? Comparison of progesterone P values among the three groups after treatmentNeither one menstrual cycle treatment nor two,the progesterone P values in the three groups were increased in various degrees than those before treatment,but no statistical difference was found(P>0.05).After three menstrual cycles,the progesterone P value of the catgut embedding and ear pressure group was significantly higher than that before treatment(P<0.01),and the progesterone P value in the conventional acupuncture group was also improved(P(P<0.05).There was a statistical difference between the catgut embedding and ear pressure group and the traditional Chinese medicine group(P<0.05).Tip:Chinese medicine treatment had poor effect for regulating the progesterone P values for patients with liver qi stagnation type of premenstrual syndrome,and after three menstrual cycles treatment,catgut embedding with auricular pressure and conventional acupuncture could increase the patient's progesterone P value.Catgut embedding with ear pressure therapy had a better effect than the traditional Chinese medicine.?The overall curative effect comparison among the thress groupsAfter three menstrual cycles treatment,the overall curative effect of the catgut embedding with ear pressure group was 86.67%,conventional acupuncture group 73.33%and traditional Chinese medicine group 70.00%.Difference among the groups was statistically significant(P<0.05),Comparing in pairs,the catgut embedding with ear pressure therapy had the best clinical effect,then the conventional acupuncture therapy,and the effect of the traditional Chinese medicine was poor.From the results of the study we can see that all of the three different Chinese medicine therapies performs a certain clinical efficacy for the treatment of liver qi stagnation type of premenstrual syndrome.In the improvement of TCM syndrome of patients with liver qi stagnation type of premenstrual syndrome,,after the three menstrual cycles,the catgut embedding combined with auricular acupressure could obviously relieve the patients' headache,irritability,breast pain,swelling,emotional depression and work ability,ect.;acupuncture treatment could also alleviate the patient's headache,breast pain,emotional depression symptoms;traditional Chinese medicine was effective in curing headache,irritability,breast pain,swelling and deprssion,etc.Comparison between the groups,the catgut embedding with ear pressure group had better clinical efficacy in curing emotional depression,low work ability and other aspects than conventional acupuncture group and traditional Chinese medicine group.SCL-90 self-rating scale,after a menstrual cycle treatment,for the patients with liver qi stagnation type of premenstrual syndrome,all of the three therapies didn't have obvious effect for the improvement of somatization symptoms,interpersonal sensitivity and psychiatric symptoms such as depression,anxiety,hostility,obsessive-compulsive symptoms,paranoia and fear.After two menstrual cycles,catgut embedding combined with auricular acupoint therapy could improve the somatization symptoms,depression and anxiety of patients with premenstrual syndrome.The treatment of Chinese medicine had only a certain effect on depressive symptoms.After three menstrual cycles,catgut embedding combined with ear pressure therapy could relieve the somatization symptoms and psychiatric symptoms such as obsessive-compulsive symptoms,depression,anxiety and paranoia in patients with premen-strual syndrome,and it was superior to conventional acupuncture group and traditional Chinese medicine group in curing depression and anxiety.However,the clinical effect of three groups were not significant for improving the interpersonal sensitivity and hostility.Three groups had no significant effect on improving estradiol(E2)level in patients with liver qi stagnation type of premenopausal syndrome.Chinese medicine treatment had poor effect for regulating the progesterone P values for patients with liver qi stagnation type of premenstrual syndrome,and after three menstrual cycles treatment,catgut embedding with auricular pressure and conventional acupuncture could increase the patient's progesterone P value.Catgut embedding with ear pressure therapy had a better effect than the traditional Chinese medicine.In summary,comaring the total effect of different Chinese medicine therapes for the treatment of liver qi stagnation type of premenstrual syndrome,the catgut embedding with ear pressure therapy had the best clinical effect,then the conventional acupuncture therapy,and the effect of the traditional Chinese medicine was poor.
