| Objective:Balloon is an important role in endoluminal treatment of femoral-popliteal arteriosclerosis obliterans.The unique design of vascutrak balloon can achieve better dilation effect and reduce vascular damage caused by dilation.This study compares the short-term efficacy of Vascutrak balloon and plain old balloon in the treatment of femorpopliteal arteriosclerosis obliterans.Methods:A total of 92 patients with femorpopliteal artery disease admitted to our department from July 2019 to October 2020 were selected,and their short-term effects after receiving corresponding treatments were retrospectively analyzed.A total of 35 patients in the vascutrak group,and57 patients in the plain old balloon group,including 59 men and 33 women,all of whom received surgery for the first time.The clinical symptoms were clearly caused by lower extremity arterial disease,and the Rutherford classification was 2-6.The lower extremity arterial CTA examination was performed before the operation to confirm the femoral popliteal artery stenosis or occlusive disease.Before surgery,relevant examinations were completed to eliminate surgical contraindications,high-risk factors and underlying diseases were controlled and adjusted,and symptomatic treatments such as improving circulation and anti-platelet were given.Two groups of patients were treated with Vascutrak balloon and plain old balloon for angioplasty.If the residual stenosis is more than 50% or there is a restrictive dissection,a salvage stent will be placed.Follow-up at 3,6,and 12 months after surgery,underwent lower extremity arterial color Doppler ultrasound and ABI examination,and recorded relevant indicators.If the condition worsened,the lower extremity arterial CTA examination was performed and corresponding treatment was given.Results:1.All patients were successfully operated.During intraoperative dilation,there was 1 case in the Vascutrak group,3 cases in the plain old balloon group with restrictive dissection,and only plain old balloon group occurred 3 cases of the residual stenosis greater than 50%,then implanted with salvage stents.The immediate postoperative diameter of the Vascutrak group was(3.5±0.4mm),while that of the plain old balloon group was(3.5±0.5mm).However,the stent placement rate and immediate postoperative diameter in the two groups was no statistical difference(P>0.05).The ABI(0.97±0.06)and its added value(0.39±0.12)of the Vascutrak group after surgery had no significant advantage compared with the ordinary balloon group(0.95±0.09,0.37±0.10).2.At 12 months postoperatively,there were 3 cases in the Vascutrak group and 15 cases in the plain old balloon group with luminal restenosis.The restenosis rate of the plain old balloon group was significantly higher than that in the Vascutrak group(26.3% vs 8.6%,P<0.05),and difference was statistically significant.The follow-up showed that at each follow-up time point,the ABI of the Vascutrak group was significantly higher than that of the plain old balloon group,and the minimum lumen diameter(MLD)was also significantly higher than that of the plain old balloon group,and late lumen loss(LLL)was significantly lower than that of the plain old balloon group,and the differences were statistically significant(P<0.05).Conclusions:In this study,the immediate effect of the Vascutrak balloon in the treatment of femorpopliteal artery disease was not significantly different from that of the plain old balloon.The results of a 12-month follow-up showed that the Vascutrak balloon was clearly superior in terms of ABI,restenosis rate,late lumen loss,and minimum lumen diameter.Therefore,the short-term curative effect of Vascutrak balloon in the treatment of femorpopliteal arteriosclerosis obliterans is significantly stronger than that of plain old balloon.Vascutrak balloon is a potential choice for keeping the lumen long-term unobstructed. |
