| Objective:To Confirm scientific hypothesis of constitution of Traditional Chinese Medicine play a unique role in the development of type2diabetes and improve the means of traditional Chinese medicine for diabetes prevention and control by Studying the risk factors for type2diabetes related literatures and clinical cases and clearing relationship among various risk factors, constitution of traditional Chinese medicine and the the onset of type2diabetes mellitus. Then put forward to combine traditional Chinese and western medicine for type2diabetes tertiary prevention, expected to improve the effect of type2diabetes prevention and treatment.Methods:Through searching "pubmed ","CNKI","VIP", and " ChinaInfo" database in both English and Chinese to find about prospective cohort study of type2diabetes risk factors related literature, we proceed meta analysis to clear the relationship between risk factors and incidence of type2diabetes. Cross-sectional study was carried out on the diabetes patients and high-risk groups based on this foundation. A preliminary conclusion of TCM constitution relationship with type2diabetes risk factors and susceptible was proposed. And then case-control study based on Chinese medicine susceptible conclusion of TCM was carried out to explore the risk factors and type2diabetes and the relationship between the TCM constitution. Setting up a system of tertiary prevention for type2diabetes mellitus combined with Chinese and western medicine.Results:1. The results of meta analysis Fixed effect model was used to analysis. Age as exposed factor, it is concluded that the OR value of2.82(2,29,3.47); Obese and overweight as exposed factor, the OR value is3.80(3.43,4.20); Smoking as exposure factor, OR value is1.28(1.21,1.21); elevated triglycerides as exposed factor, OR value is2.41(2.00,2.90); High-density lipoprotein cholesterol as exposed factor, OR value is1.38(1.12,1.69);Elevated systolic blood pressure as exposed factor, OR value is3.53(3.24,3.84); Diastolic blood pressure as exposed factor, OR value is3.37(3.17,3.59); Ffasting blood glucose as exposed factor, obtain the OR value of20.65(18.04,23.63); Abnormal glucose tolerance as exposed factor, the OR value is11.48(9.38,14.05); Glycated hemoglobin5.5%as the point of tangency, obtain the OR value is8.79(7.52,10.28);Metabolic syndrome as the exposure factor, it is concluded that the OR value of5.98(4.96,7.20].; First-degree relatives with diabetes as the exposure factor, OR value is2.20(1.93,2.51);2. Type2diabetes risk factors and the relationship of TCM constitution Comparing with the control group and within group by changing physical data into measurement data, phlegmy wet body play an important role in all the major risk factors crowd. Constitution tended to qi deficiency and Yang deficiency constitution when the age was more than40; Smoking group tended to phlegmy wet and damp heat constitution; Drinking group tended to phlegmy wet and damp heat constitution; Less exercise groups tended to phlegm dampness-constitution; High blood glucose group tended to phlegm dampness constitution; Dyslipidemia groups tended to phlegm dampness constitution; Overweight or obese tended to phlegm dampness constitution; Disease of heart head blood-vessel groups tended to phlegm dampness constitution; First-degree relatives of diabetes groups tended to phlegm dampness constitution; First-degree relatives disease of heart head blood-vessel groups tended to phlegm dampness and blood stasis constitution.3. Based on the traditional Chinese medicine susceptible case-control studies of type2diabetesLayered case-control study of risk factors of people was carried out by changing the physical data into count date. Comparing to control group, qi deficiency constitution, phlegmy wet physical associated with the onset of diabetes, the phlegm wet constitution as a risk factor for diabetes, OR=2.994. People with one risk factors as control group, the peace constitution, qi deficiency constitution, Yang deficiency constitution, phlegmy wet constitution, the great mass associated with the onset of diabetes, the phlegm wet constitution as a risk factor for diabetes, OR=1.947. People with two kinds of risk factors as control group, the peace constitution, qi deficiency constitution, damp heat constitution, phlegmy wet body constitution, the great mass associated with the onset of diabetes, the phlegm wet constitution as a risk factor for diabetes, OR=1.842. People with three kinds of risk factors as the control group, qi deficiency constitution, phlegmy wet physical, damp heat constitution is associated with diabetes, the phlegm wet constitution as a risk factor for diabetes, OR=1.883; People with With four or more risk factors as control group, peace constitution, damp heat constitution, special talents and physical associated with the onset of diabetes, the OR value is less than1.4. Type2TCM susceptible multiariable logistic regression analysisLogistic regression analysis showed that gender, age, nationality, educational background, first-degree were relatived with diabetes, smoking, not early to bed and early to rise, makes food partial heavy, less exercise, the body poor adapt to seasonal change were associated with the phlegmy wet constitution. People who have received a higher education, first-degree relatives with diabetes, partial addicted to smoking, diet, exercise, body poor adapt to seasonal change were the risk factors of.formation of phlegmy wet constitution.Conelusion:Age, smoking, lack of exercise, high blood pressure, sugar regulation damaged, dyslipidemia, overweight or obese, cardiovascular and cerebrovascular diseases, first-degree relatives with diabetes and cardiovascular disease are risk factors for type2diabetes. It is closely related between the TCM phlegm wet body constitution, risk factors and type2diabetes.correcting phlegmy wet constitutiont and geting rid of modern medical risk factors will play an important role in preventing type2diabetes. combining with Chinese and western medicine to type2diabetes tertiary prevention has a bright future. |
