With the development of medical informatics, the medical information systems have become more and more large-scale, more and more specialized. There are hundreds of heterogeneous devices and information systems in a typical hospital. In order to meet the information needs of health care facilities, the information from various types of devices and medical information systems must be integrated to support all type of medical services. On the other hand, it's necessary to establish the overall health care collaborative environment for all the heterogeneous devices and information systems, improving efficiency and quality of medical services. To achieve this, the integration between various types of heterogeneous devices and information systems is the key, but the lack of interoperability standards, the lack of a comprehensive standard terminology, the lack of plug and play software applications, the lack of clear cost/benefit evidence and other issues has been hampering the process of integration. With the emergence of new mode of combining hospital and community or hospital group, the integration of medical resources between the hospitals has also been put on the agenda. The integration becomes more compelling and more complex.In order to present a total solution to overcome the medical information system integration problem, this dissertation aims at how to make and achieve the integration goals. On the one hand, an interoperability maturity model is proposed to facilitate the assessement of the current integration status and facilitate the goals setting. On the other hand, an integration model, an integration engine and an implementation method are proposed to achieve the integration goals.The main contributions of this dissertation include:1,Reviewed the recent progress of medical information systems integration systematically. Made a thorough analysis of the integration requirements of medical information systems.2,A medical information systems interoperability maturity model MISIM is proposed, which can guide the hospital managers to assess the interoperability level between medical information systems, to make goals for next medical information system integration practice.3,A hierarchical medical information system integration model SWIM (Semantic and Workflow based Integration Model) is proposed to achieve the semantic and process interoperability between medical information systems. System linking layer is designed to support various health information exchange protocols. Syntax and semantic converting layer are designed to fullfil the mutual data transformation and translation respectively between heterogeneous systems. Transaction mapping layer is desigened to achieve the mapping between messages and standard trigger events. Process management layer is designed to handle the issues related about workflow.4,A medical information systems integration engine is designed and implemented based on the SWIM. Conversion and coordination services for interfaces, datas, functions and processes are fulfilled via information bus interfaces to address the data integration and workflow integration issues between heterogeneous medical information systems.5,Finally, a system integration practice is carried out successfully in a large-scale general hospital and a medical information system integration method is formed. The integration method is the guidance for the orderly planning and implementation of the medical information system integration.The result of system integration practice shows that the medical information systems integrated via engine successfully. The systems with different interface can exchange messages. The information is integrated at semantic level which has opened up broad prospects for the application of clinical data. The process integration ensures seamless collaboration across different systems to meet the needs of the user process.
