| Objective and Significance:The disease of fullness and oppression is the common ailment of spleen and stomach system of TCM. The weakness of spleen and stomach is the significant pathogenesis. According to the symptom characteristics of this kind of disease, it equals to the chronic superficial gastritis or the functional dyspepsia of the Western medicine.Most scholars learn that the patients of fullness and oppression have the different type of gastrointestinal dyskinesis according to the different pattern of syndrome of TCM and that the patients of weakness of spleen and stomach have the characteristics of Bradygastria according to the research of the gastric electrical parameters, MMC and gut. Their migarating motor complexs manifest the shorter II and III phase, extending I phase. The patients of weakness of spleen and stomach have the lower motilin level obviously compare with the healthy people group. They show the inferior gastrointestinal motivation. The disease of chronic superficial gastritis belongs to the category of fullness and oppression or gastrodynia of TCM. The most patients of CSG often have the symptoms of dyspepsia such as the vague pain of superior belly, the decreased food appetite,post-meal abdominal mass,sour regurgitation, etc.The domestic and international research make clear that patient mostly have the delayed gastric emptying and that the inferior gastrointestinal motivations is their main patho-changes. The gastrointestinal electrical derangement and the abnormal slow wave is the pathophysiologic base of the gastrointestinal motivations disorder. The reaearches on the gastric electrical parameters and the blood plasma motilin show that the levels of the blood plasma motilin correlate positively with the average amplitude of vibration and frequency of the gastric electrical parameters. The blood plasma motilin levels of these patients descend obviously accompanying with the delayed gastric emptying. Therefore the researching of invigorating spleen and regulating qi decoction treating with the fullness and oppression patients of spleen and stomach weakness group can offer the foundation for treating this kind of disease with the advancing methods of TCM and can offer the study case and data for the advancing developing and researching this invigorating spleen and regulating qi decoction.Methods:This thesis includes two parts:literature review and clincal research. The literature review expound from two aspects of traditional Chinese medicine and western medicine about the studying progression of the disease of the fullness and oppression, the CSG, the gastric electrical parameters and the correlation factors of gastrointestinal motivation. The clinical research contain the curative effect of the Invigorating spleen and regulating qi Decoction for the syndrome cluster of the weak spleen and stomach, the curative effect of the gastric electrical parameters improving condition, the curative effect of the correlation factors improving condition of gastrointestinal motivation in blood plasma of the fullness and oppression patients of the spleen and stomach weakness group. Combinating above researching aspects, we can observe the curative effect of the Invigorating spleen and regulating qi Decoction for the syndrome cluster of the weak spleen and stomach and promoting gastrointestinal motivations. The observed group and the control group were established. The main study methods are the random and the contrast principles.Results:The literature review clarify the history origin and development, etiological factors and pathogenesis, determination of treatment based in pathogenesis obtained through differentiation of symptoms and signs of the fullness and oppression. The etiological and pathological factors, the characteristics of gastrointestinal motivation of CSG were clarified. The studying progression of the gastric electrical parameters and the correlation factors of gastrointestinal motivation such as MTL, CGRP and NO were also clarified. The literature review found the clue and base to study the disease.The curative effect of the Invigorating spleen and regulating qi Decoction for the syndromes cluster of the weak spleen and stomach:the curative effect of the curing intermediate stage(two weeks):There are different degree improving in the syndromes of the mass of gastric cavity or abdominal distension, ructation and sour regurgitation, fewer foods and anorexia, coated tongue, pulse tracings in the observed group and control group(P<0. 01-P<0. 05). The curative effect of the Invigorating spleen and regulating qi Decoction group is better than the Decoction of Costus and Amomum with Six Noble Ingredients group(P<0. 05). The curative effects of the rests of the syndromes cluster in both groups are not obvious. The curative effect of the curing whole range (four weeks) :There are significantly improving in the syndromes clusters of the Invigorating spleen and regulating qi Decoction group except the syndrome of nausea and vomitting comparing with the prior treatment stage and the curing intermediate stage (P< 0.01-P < 0. 05). There are significantly improving in the syndromes clusters of the Decoction of Costus and Amomum with Six Noble Ingredients group except the syndrome of nausea and vomitting, ructation and sour regurgitation, etiolating vultus, powerless defecation comparing with the prior treatment stage and the curing intermediate stage (P <0. 01-P<0. 05). The curative effects of the Invigorating spleen and regulating qi Decoction for the syndrome of mass of gastric cavity or abdominal distension and ructation and sour regurgitation surpass highly significantly the Decoction of Costus and Amomum with Six Noble Ingredients group(P<0. 01). The curative effects of the Invigorating spleen and regulating qi Decoction for the syndrome of afterfood abdominal distension aggravating, gastric cavity pain, tasteless mouth and hydroadipsia, fewer foods and anorexia, texture of tongue,coated tongue and pulse tracings significantly surpass the Decoction of Costus and Amomum with Six Noble Ingredients group(P<0. 05). There are different degree improving in the syndromes of the mass of gastric cavity or abdominal distension, ructation and sour regurgitation, fewer foods and anorexia, coated tongue, pulse tracings in the observed group and control group. In the four weeks the longer the course of treatment is, the better the curative effect is. This results indicate that the curative effect of the Invigorating spleen and regulating qi Decoction for the patients' syndrome clusters of the weak spleen and stomach is relevant with the course of treatment. After the four weeks curing course, the recovery rate of the syndromes cluster is 31.3%,the excellence rate of the syndromes cluster is 46. 9%, the utility rate of the syndromes cluster is 18. 8%, the inefficacy rate of the syndromes cluster is 3.1% in the Invigorating spleen and regulating qi Decoction group. In the Decoction of Costus and Amomum with Six Noble Ingredients group the recovery rate of the syndromes cluster is 22.6%, the excellence rate of the syndromes cluster is 22.6%, the utility rate of the syndromes cluster is 45. 2%, the inefficacy rate of the syndromes cluster is 9. 7%. On the aspect of the curative effects, the Invigorating spleen and regulating qi Decoction group surpasses the Decoction of Costus and Amomum with Six Noble Ingredients group (P<0. 05). In the Invigorating spleen and regulating qi Decoction group the general improvement rate is 96.9% and the inefficacy rate is 3.1%. In the Decoction of Costus and Amomum with Six Noble Ingredients group the general improvement rate is 90. 3% and the inefficacy rate is 9. 7%. Both decoctions have effective curative effects. There are no significant differences on the total effective rates of two groups.The curative effect of the Invigorating spleen and regulating qi Decoction for the gastric electrical parameters of the fullness and oppression patients:The DF, DP, N% of the ante-meal gastric electrical parameters in both groups step up significantly in the post treatment comparing with the prior treatment, nevertheless, the ante-meal T%, B%, DFIC in both groups descend significantly in post treatment comparing with the prior treatment. The DF, DP, N% of the post-meal gastric electrical parameters in both groups step up significantly in the post treatment comparing with the prior treatment, nevertheless, the ante-meal T%, B%, DFIC, DPIC in both groups descend significantly in post treatment comparing with the prior treatment. Through the group comparison, we can draw the conclusions that the observed group' s ante-meal DP, N% in the post treatment step up significantly comparing with the control group(P<0. 01), nevertheless, the ante-meal T%, B%, DFIC in the observed group descend significantly comparing with the control group(P<0. 01-P<0. 05). There is not statistically significant in the group comparison of DF, DPIC. Through the group comparison, we can draw another conclusion that the observed group' s post-meal DP, N% in the post treatment step up significantly comparing with the control group(P<0. 01), nevertheless, the post-meal T%, B%, DFIC, DPIC in the observed group descend significantly comparing with the control group (P <0. 01-P<0. 05). Combining the above research data, we can know that not only the Invigorating spleen and regulating qi Decoction can significantly improve the patients' gastric electrical rhythm, but also its curative effects are better than the Decoction of Costus and Amomum with Six Noble Ingredients groups' after the treatment period of four weeks.The curative effect of the Invigorating spleen and regulating qi Decoction improving on the correlation factors of gastrointestinal motivation in blood plasma of the fullness and oppression patients:The densities of MTL in blood plasma in both groups step up significantly in the post treatment comparing with the prior treatment (P<0. 01), nevertheless, the densities of CGRP, NO in blood plasma in both groups descend significantly in post treatment comparing with the prior treatment(P<0. 01-P<0. 05). Through the group comparison, we can prove that the density of MTL in blood plasma in the observed group is highly significantly higher than the control group in the post treatment(P <0. 01), howerer, the density of NO in blood plasma in the observed group is significantly lower than the control group in the post treatment (P<0. 05). We can also prove that the density of CGRP in blood plasma in the observed group is highly significantly lower than the control group in the post treatment(P < 0.01).Combining the above research data,we can draw the conclusion that not only the Invigorating spleen and regulating qi Decoction can significantly heighten the patients' density of MTL in blood plasma, but also can significantly cut down the density of NO and CGRP in blood plasma after the treatment period of four weeks. Furthermore these indexes' degrees of heighening and stepping down of the Invigorating spleen and regulating qi Decoction group are more notable than the Decoction of Costus and Amomum with Six Noble Ingredients groups' .Conclusions:The clinical research results show that both the drugs of the observed group and the control group have the satisfactory the therapeutical effect.Both decoctions have the distinct curative effects to the patients of fullness and oppression of weakness of spleen and stomach type for the observed indexes of the syndrome clusters of the weak spleen and stomach type, the gastric electrical rhythms, the correlation factors of gastrointestinal motivation in blood plasma. Furthermore the curative effects of the Invigo- rating spleen and regulating qi Decoction are super than the Decoction of Costus and Amomum with Six Noble Ingredients groups' .The topical results confirm that the curative effects are certain to the disease of fullness and oppression of weakness of spleen and stomach type with the Decoction of invigorating spleen and regulating qi. The improvements of the syndrome clusters of TCM are equal to the improvements of the gastric electrical parameters and the improvements of the correlation factors of gastrointestinal motivation in blood plasma of Modern Medicine. Therefore it can be inferred that the one of the mechanisms of treating the disease of fullness and oppression of weakness of spleen and stomach type with the Decoction of invigorating spleen and regulating qi is improving the patients' gastric electrical parameters and correlation factors of gastrointestinal motivation in blood plasma.
