Clinical Study On The Treatment Of Early Stage Of Diabetes With The Method Of Invigorating Stomach And Invigorating Spleen

Objective:1.To observe the clinical effect of the method of purging stomach and invigorating the spleen to treat the patients with prediabetes.2.to investigate the influence of the method of purging stomach and reinforcing spleen on blood glucose and lipid metabolism indexes and insulin resistance in patients with prediabetes.3.to explore the pathological mechanism of the intervention of purging stomach and invigorating the spleen on prediabetes.Method:Methods:60 patients were divided into acupuncture group(32 cases)and control group(28 cases)according to the random number table.The control group was treated with health education,diet control,reasonable exercise and simple lifestyle intervention.The acupuncture treatment group was given the principle of purging stomach and invigorating the spleen on the basis of the scientific lifestyle intervention.The main spleen Yu,Zhongwan,Zusanli,Sanyinjiao,Taibai,neiting.The first stab Pishu,after the surplus point.Zhongwan,Zusanli and Neiting with diarrhea,the first reducing method of lifting and thrusting,light thrusting and twirling needle 6 times,The thumb,index finger forward backward twist,line 6.Pishu,Taibai,Sanyinjiao with reinforcing method,first mentioned interpolation,re inserted light,line 6,twirling reinforcing method,the index finger thumb forward,backward,line 6 times,Yu Xueping reinforcing reducing needle for 30 min.3 times a week,once every other day,two groups are 12 weeks intervention.The score of syndrome before and after treatment was recorded,detected the fasting blood glucose(FPG),glycosylated hemoglobin(HbAlc),total cholesterol(TC),triglyceride(TG),high density lipoprotein(HDL-C),low density lipoprotein(LDL-C),fasting insulin(FINS),fasting serum C peptide(FCP),leptin(LEP),resistin(RETN),glucagon like peptide-1(GLP-1),insulin resistance index(HOMA-IR),insulin sensitivity index(ISI)of pancreatic islet beta cell function index(HOMA-).Evaluate curative effect.Results:1 The distribution of clinical efficacy between the two groups was statistically significant(P<0.05).There were significant differences between the two groups in symptom scores before and after the intervention(P<0.05).Compared with the control group,the total symptom scores of the acupuncture group were significantly lower than that of the control group,the difference was statistically significant(P<0.05).Before treatment,more eating,thirst,polydipsia,fatigue,constipation or diarrhea symptoms,the greater the proportion of points,acupuncture treatment of the above symptoms integral reduction effect is obvious.2.Compared with before intervention,the difference of FPG,HbAlc,FINS and FCP between the two groups was statistically significant(P<0.05).The decrease of above indexes in acupuncture group was significantly higher than that in control group(P<0.05)3.Compared with before intervention,the difference of ISI and HOMA-IR between the acupuncture group and the intervention group was statistically significant(P<0.05),but there was no significant difference between the control group and the intervention group(P>0.05).The difference of ISI and HOMA-IR in the acupuncture group was significantly higher than that in the control group.4.Compared with before intervention,the intervention of TG and LDL-C acupuncture group had significant difference(P<0.05),the control group after the intervention of lipid metabolism index had no significant difference(P>0.05),acupuncture group of TG,HDL-C,LDL-C better than the control group.5.Compared with intervention,the difference of RETN and GLP-1 between the acupuncture group and the intervention group was statistically significant(P<0.05),and there was no significant difference between the control group,RETN,LEP and GLP-1 before and after intervention(P>0.05).The difference of RETN and GLP-1 between the acupuncture group and the control group was significantly higher than that in the control group(P<0.05).Conclusion:1.Lifestyle intervention has an effect on the improvement of glucose and lipid metabolism in patients with prediabetes,but its effect is lower than that of acupuncture.2.The syndromes of patients with diabetes with strong stomach spleen weak performance,spleen and stomach purging method can alleviate the clinical symptoms,correct glucose and lipid metabolism,insulin secretion,increase insulin sensitivity,improve insulin resistance and its mechanism may be related to the down-regulation of RETN level,increase the peripheral tissue glucose utilization,upregulate the expression of GLP-1 and promote beta cell proliferation,reduced beta cell apoptosis.3.Conclusion stomach purging and invigorating spleen therapy is an effective treatment for symptomatic patients with prediabetes.
