The main cause of diabetes complications is hyperglycemia which leads to injury blood vessels in all the body. Diabetic foot is one of severe complication in diabetes mellitus. Furthermore, diabetic foot is also a major cause of disability and death, it became an impending illness threatening quality and quantity of human life.Using traditional Chinese medicine (TCM) to treat vascular disease in lower extremity-diabetic foot has given high efficiency and original advantage. But there are still shortcomings which haven't been solved yet such as too much signs of traditional Chinese medicine, uncommon consent in the standards of pattern identification, difficult to carry out medical exchanges and collective experiences so that they reduce development of treatment by TCM.Objective:Based on clinical observations, gathering case-records in peripheral vascular disease in diabetic foot, research Western and Chinese medicine documents in the same field, finding characteristic signs of diabetic foot and relative elements, establishing criterial and unprejudiced traditional Chinese pattern identification of diabetic foot, making progress to perfect clinical syndrome differentiation and treatment for diabetic foot, thenceforward dignifying knowledge of traditional Chinese diagnostics and modality of treatment for diabetic foot in Institute of Traditional medicine in Hochiminh City.Method:Aspect of basic theory:Collecting and summarizing Western and Chinese medicine documents in diabetic foot in many respect:definition, epidemiology, patients with muscular disease (benh nhan benh ca), damage factors influence wound and gangrene, achievements and results of studies about diagnostic criterion and methods of treatment.Aspect of clinical study:Cross-sectional study. Collecting200diabetic records in Hochiminh city, Vietnam; carry out statistic, analysis and summarization clinical symptoms; sign in local and all over; pulse and tongue diagnosis; pattern identifications and signs, examination results.Results:1. Finding vascular disease in diabetic foot early is very important because there are a part of patients be treated in long-term but their glycemia, HbA1C, blood lipid are still in high level; especially triglyceride raise too much up to2000mg/dl and more. It demonstrates that patients be treated in long-term don't get results as requirement, even lead them to diabetic complication in future. 2. Based on observing femoral artery, popliteal artery and dorsalic pedis artery by color-Doppler ultrasound, the results are narrow intravascular diameter, decreasing cardiac output, augmentative difference of speed (statistic:p-value<0.05). These results are similar to some results in china and other countries.3. A part of patients be treated in long-term in Vietnam still have glycemia, HbA1C, blood lipid in high level because their basic diabetic knowledge is so bad. The reason is the majority of patients whose ages are fairly old (60.75±12.29) with60%of them living in countryside so they aren't able to have condition to approach exactly information about diabetes treatment and prevention. Besides, the number of diabetic patients is too much not enough time to be examined and consulted, besides, there is lack of combination between Western and Chinese medicine, as well as omittance of psychology therapy.4. Color-Doppler ultrasound isn't able to diagnose diabetic foot disease in the early stage.5. Ratio of chronic venous insufficiency is so high (98.5%).Conclusion:The prevalence of diabetic patients in Vietnam increase more and more. Preventing complications of diabetes, especially improving awareness of Diabetic foot, studying methods of prevention and treatment are very important.
