Objective:To study the clinical effects and safety of the nursing methods of foot reflex zone massage and Chinese herb pediluvium in preventing and treating diabetic foot of mid-and-high risk and explore its mechanism.Methods:Adopting experimental methods of random and control, total of 90 eligible patients were randomly assigned to three different groups, namely group A(treated with individual healthy education and feet nursing education based on conventional therapy of diabetes mellitus), group B (treated with foot reflex zone massage based on group A) and group C (treated with Chinese herb pediluvium based on group B). Each group included 30 cases and therapeutic course lasted for 8 weeks. Then the ankle brachial index (ABI) and toe brachial index (TBI) of the lower limbs, the integral of symptom and signs, the clinical effect and the self-care ability in each group were observed and compared. All data were analyzed with SPSS 16.0 and P<0.05 means that there were statistic differences in 3 groups.Results:(1)After nursing intervention, ABI and TBI of the lower limbsin group C were better than that in group A and B (P<0.05). (2)The integral of symptom and signs in group B and C were better than that in group A (P<0.05) and the difference between group B and C was indistinctive (P>0.05). (3)The markedly effective rate in group C and B were 66.67% and 53.33% seperately, which was obviously higher than that in group A (6.67%) (P<0.05).However, the difference between group B and group C was indistinctive (P>0.05). (4)The average score of each aspect in group C was much higher than that in group A and B (except exercise), and there was statistic difference in feet nursing, diet control and the treatment of abnormal blood glucose among 3 groups (P<0.05) Conclusion:(1)The program can obviously improve ABI and TBI of patients with diabetic foot of mid-and-high risk. (2)The program can obviously improve the symptom and signs of patients with diabetic foot of mid-and-high risk. (3)The program can enhance the self-care ability. (4)Through the dual effects of drugs and heat by Chinese herb pediluvium, and the whole regulating effect by foot reflex zone massage, the program plays an important role in treating and preventing diabetic foot of mid-and-high risk. (5)The program reflects the TCM theory of Zhiweibing, and has a certain superiority in the treatment and prevention of diabetic foot of mid-and-high risk.
