Objective: By observing the changes of VAS score,TCM syndrome score and dysmenorrhea symptom score,the short-term and long-term effects of Ciliao acupoint catgut embedding combined with Gexia Zhuyu decoction in the treatment of primary dysmenorrhea of Qi stagnation and blood stasis type were discussed,providing more effective clinical basis and treatment ideas for the clinical treatment of female primary dysmenorrhea by TCM.Materials and methods: According to the established inclusion and exclusion criteria,60 patients with primary dysmenorrhea of Qi stagnation and blood stasis type were randomly divided into control group and observation group,with 30 cases in each group.Both the control group and the observation group were treated with Gexia Zhuyu Decoction,which was taken orally 7 days before menstruation until the end of menstruation.One menstrual cycle was one course of treatment,and three courses of treatment were taken continuously;The observation group was treated with catgut embedding at Ciliao points on both sides on the basis of traditional Chinese medicine,and catgut embedding was performed once 5-7 days before menstruation for 3 consecutive menstrual cycles.The VAS score,TCM syndrome score and dysmenorrhea symptom score were used as the observation indicators of the efficacy of this trial,and were evaluated before treatment,three courses of treatment,and three months after treatment.The results of the two groups were compared and analyzed by using SPSS27.0 statistical software.Results:1.General situation: There was no significant difference(P>0.05)in age,menstrual cycle,VAS score before treatment,traditional Chinese medicine syndrome score,and dysmenorrhea symptom score between the two groups of patients,indicating comparability.2.After treatment,all scores in the observation group decreased compared to before treatment(P<0.05);The VAS score,dysmenorrhea symptom score,and traditional Chinese medicine syndrome score in the control group showed a decrease compared to before treatment in terms of lower abdominal distension and pain,blood purple dark with blood clots,breast distension and pain,and chest tightness(P<0.01),while there was no statistically significant difference in terms of less menstrual volume,tongue purple dark or with ecchymosis,and pulse string astringency(P>0.05);The difference between groups was statistically significant(P<0.05),and the therapeutic effect of the observation group was better than that of the control group.3.After 3 months of treatment,follow-up was conducted,and all scores in the observation group decreased compared to before treatment(P<0.05);The VAS score,dysmenorrhea symptom score,and traditional Chinese medicine syndrome score in the control group showed a decrease compared to before treatment in terms of lower abdominal distension,less menstrual flow,purple and dark blood with blood clots,breast distension,chest tightness,purple and dark tongue,or ecchymosis(P<0.05),but there was no statistically significant difference in pulse stringency(P>0.05);The observation group had more advantages in reducing VAS score,dysmenorrhea symptom score,improving menstrual flow,dark blood with blood clots,breast swelling and pain,and chest tightness in terms of long-term efficacy.The difference between the groups was statistically significant(P<0.05).4.Clinical efficacy comparison: After three courses of treatment,the total effective rate of the observation group was 96.7%,while the total effective rate of the control group was 90.0%.There was no statistically significant difference between the groups(P>0.05),and the recent efficacy of the two groups was similar;After 3 months of treatment,the total effective rate of the observation group was 93.3%,while the total effective rate of the control group was76.7%,with a statistically significant difference(P<0.05).The long-term efficacy of the observation group was better than that of the control group.Conclusion:1.For the treatment of primary dysmenorrhea of Qi stagnation and blood stasis type,the combination of Ciliao acupoint catgut embedding and Gexia Zhuyu decoction therapy,as well as the simple use of Gexia Zhuyu decoction therapy,have good therapeutic effects.Both can reduce the VAS score and dysmenorrhea symptom score of such patients,improve lower abdominal pain,blood purple and dark with blood clots,breast pain,and chest tightness.The recent therapeutic effects of the two groups are similar.2.Compared to simply using Gexia Zhuyu Tang for treatment,Ciliao acupoint catgut embedding combined with Gexia Zhuyu Tang has a more significant long-term effect in reducing VAS score,dysmenorrhea symptom score,and improving menstrual flow,blood purple and dark with blood clots,breast pain,and chest tightness.It can reduce the frequency of patients coming to the hospital for treatment,facilitate promotion,and has high clinical application value.
