Objective: To observe the clinical efficacy of point catgut embedding combined with pricking and cupping therapy in the treatment of primary dysmenorrhea with cold stagnate and blood stasis,and to compare the efficacy difference of the two therapies in the treatment of this disease with oral ibuprofen sustained release capsule,providing clinical basis for the treatment of this disease with acupoint catgut embedding combined with pricking and cupping therapy.Methods: 72 patients with PD(cold stagnate and blood stasis type)meeting the entry criteria were randomly divided into 2 groups.The combined group(36 cases)was treated with point catgut embedding combined with pricking and cupping therapy,once a month 7 days before menstruation,for one course of treatment;The western medicine group(36 cases)was treated with ibuprofen sustained-release capsules orally,1 capsule(0.3g)/time,Twice a day,Take it for 3 days or stop taking it when the pain and discomfort improves,1 menstrual cycle for 1 course of treatment;After 3 courses of treatment,the changes of visual analog scale(VAS),Cox Menstrual Symptom Scale(CMSS),total TCM syndromes scores and each of single syndrome were observed in the two groups before and after stopping treatment,as well as the 3-month recurrence after stopping treatment were statistically analyzed,so as to evaluate the clinical efficacy of point catgut embedding combined with pricking and cupping therapy in the treatment of primary dysmenorrhea with cold stagnate and blood stasis.Result:1.Clinical total effective rate: After treatment,the total effective rates of the combined group and the western group were 94.28% and 74.29% respectively,and the combined group was better than the western group(P<0.05).2.VAS: After treatment,the score in both groups was decreased compared with that before treatment(P<0.01),and the reduction in combination group was more significant compared with western medicine group(P<0.05).3.CMSS: After treatment,the total duration and severity of dysmenorrhea symptoms in both groups were decreased compared with those before treatment(P<0.05),and the reduction in combination group was more significant than that in western medicine group(P< 0.05).4.TCM syndrome:(1)Total integral: After treatment,the total score of TCM syndrome in2 groups decreased compared with that before treatment(P<0.01),and the decrease amplitude was greater in the combined group(P<0.01);(2)Individual integral of each characteristic syndrome: In the combined group,7 typical symptoms of cold coagulation and blood stasis were all decreased compared with before treatment(P<0.01),while in the western medicine group,5 symptoms of cold pain in lower abdomen,less obstruction of menstruation,pale face,nausea and vomiting,diarrhea were decreased compared with before treatment(P<0.01).In addition,the improvement effect of combination group on cold pain in lower abdomen,dark coldness of blood,cold limbs and diarrhea was stronger than that of western medicine group(P<0.05).5.Recurrence rate and safety :(1)The recurrence rate of the combined group(9.09%)was lower than that of the western group(30.77%),and the difference was statistically significant(P<0.05);(2)The incidence of adverse reactions was 5.71% in the combined group and17.14% in the western medicine group,but the difference was not statistically significant(P>0.05).Conclusion:1.Both oral ibuprofen sustained release capsules and acupoint catgut embedding combined with cupping therapy can effectively treat PD with cold coagulation and blood stasis,shorten the duration of each symptom and reduce the severity,but point catgut embedding combined with pricking and cupping therapy is more effective,and the recurrence rate is lower,with better safety.2.Acupuncture point embedding combined with acupuncture and cupping can significantly improve various TCM syndroms of PD patients with cold stagnate and blood stasis,and has an overall regulating effect.
