Purpose:1.To explore the theoretical origin of Buzhong Yiqi Decoction in the treatment of cardiovascular diseases by systematically combing ancient literature.2.To investigate the distribution of Qi deficiency in coronary heart disease syndromes to provide clinical basis for the application of Buzhong Yiqi Decoction.Material and method : Using the Chinese Medical Canon,with "Buzhong Yiqi decoction" as the key word to search,the retrieved items were screened,sorted and analyzed,and SPSS software was used for statistical analysis.Taking "traditional Chinese diagnostics" as the inquiry standard,the medical history of inpatients in the Department of Cardiology of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from September 2021 to September 2022 was collected,and 95 cases were screened according to the inclusion and exclusion criteria.The collected data were collated and analyzed,and SPSS software was used for statistical analysis.Results:1.By using the Chinese Medical Canon and taking "Buzhong Yiqi Decoction" as the key word,a total of 8295 articles were retrieved,and 378 valid ancient literature data were obtained after screening.The selected literature mainly covered the category of medical books included,dynasty involved,composition,dosage,processing method,preparation method,decoction method,administration method,indications,tongue and pulse.(1)The categories of medical books included: a total of 93 ancient Chinese medical books recorded the recipe,including 56 clinical cases,16 formularies,7 medical theories and cases,6 cold damage diseases,6 warm diseases,and 2 health preservation.(2)Dynasties: Song Dynasty included 4 ancient books and 13 document entries,Ming Dynasty included 28 ancient books and 138 document entries,Late Ming Dynasty and Early Qing Dynasty included 1 ancient book and 3 document entries,Qing Dynasty included 60 ancient books and 223 document entries.(3)Drug composition: 182 records(48.15%)were consistent with the original prescriptions,and other records were added or subtracted.(4)Drug dosage: There were 8 items with the same drug dosage as recorded in "Treatise on Clarification of Perplexities About Internal and External Damage",and the other records all increased the drug dosage to varying degrees on the basis of the original prescription.(5)Processing methods:Drugs with more combinations are Licorice(322),Astragalus membranaceus(238),Atractylodes macrocephala(220)and Angelica sinensis(94),and Licorice is mostly burnt licorice,which is consistent with the original formula.Astragalus is mainly broiled with honey,Atractylodes are mainly scrambled,and Angelica is mostly processed with wine.(6)Preparation methods: A total of 18 preparation methods were recorded,including 12 files,5chews and 1 fine cutting.(7)Decoction methods: There were 308 records of decoction methods,including 278 items of "water decoction",207 items of "ginger and jujube decoction",37 items of "removing residue after decoction" and 6 items of "ginger decoction",of which 117 items were basically consistent with the original recipe.(8)Methods of administration: 277 items were recorded,including 135 items taken on an empty stomach,64 items taken on a hot diet,53 items taken on a warm diet,20 items taken in the afternoon,8 items taken far after food,7 items taken on a slightly hot diet,5 items taken after breakfast,and 2 items taken after food.(9)Indications: systemic symptoms(128),dietary taste(109),body and limb skin(94),stool and discharge(92),head and neck features(88),speech sound and odor(54),male and female symptoms(50),chest,back,waist,abdomen and anus(43).(10)Tongue and pulse: of 378 articles,only 3 articles mentioned tongue image,2 articles were stiff tongue body,1 article was white and slippery fur,and 48 articles mentioned pulse image,9 articles related to the original text "surging and large pulse",9 articles related to "pulse deficiency",and the rest of the pulse image was mainly vacuous pulse,fine pulse,faint pulse,floating pulse,and weak pulse.2.A total of 95 patients hospitalized in the First Department of Cardiology of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from September 2021 to September 2022 were collected.The ages ranged from 47 to 93 years old,including 44 males and 51 females.(1)The main clinical symptoms of the patients were oppression in the chest,shortness of breath,chest pain,lack of strength,palpitations,inability to sleep,dry mouth,etc.Patients’ tongue manifestations mostly showed pale,dark red tongue,thin white fur,little fur,white slimy fur,etc.,and their pulse condition mostly showed string-like slippery pulse and fine weak pulse.(2)Symptoms of qi deficiency syndrome included shortness of breath(88.42%),lack of strength(69.47%),lassitude of spirit and lazy speech(48.42%),spontaneous sweating(44.21%),dizziness(41.05%),weak tongue(38.95%),faint low voice(32.63%),and faint face(17.89%).Patients generally had shortness of breath and a lack of strength.(3)Clinical symptoms were classified into 4 categories by K-means cluster analysis,including34 cases of qi deficiency syndrome(35.79%),21 cases of qi deficiency syndrome with blood stasis(22.11%),15 cases of qi and yin dual deficiency syndrome(15.79%),and 25 cases of phlegm and blood stasis syndrome(26.32%).There is a large proportion of QI deficiency in the entire patient.(4)There were 75(78.95%)patients with abnormal cardiac ultrasound results,which generally showed abnormal heart function.Conclusion:1.Buzhong Yiqi decoction has been widely used clinically in ancient times,with definite efficacy,and can be flexibly applied to various diseases with symptoms of Qi deficiency.2.Clinical patients with coronary heart disease usually have symptoms of Qi deficiency,which are basically consistent with symptom manifestations of diseases treated by Buzhong Yiqi decoction as recorded in the literature,and the frequency of the deficiency manifestations that are consistent with it is located in the front.3.Buzhong Yiqi decoction has certain advantages and application value in the treatment of coronary heart disease with qi deficiency characteristics,and is worthy of further promotion and application in modern clinical medicine. |
