Objective: To investigate the effects of Taohong Siwu Decoction on traumatic femoral head avascular necrosis in rats based on Osterix transcription factor,and to further clarify the efficacy and mechanism of Taohong Siwu Decoction through experiments,to further clarify the pathogenesis of traumatic femoral head necrosis,and to provide experimental data for the clinical diagnosis and treatment of traumatic femoral head necrosis.Methods:1.Thirty-five healthy SD rats were selected as experimental subjects and randomly divided into five groups: taohong Siwu Decoction high-dose group,medium-dose group,low-dose group,model group and normal group,with 7 rats in each group.2.The rats in taohong Siwu Decoction high-dose group,Taohong Siwu Decoction medium-dose group,Taohong Siwu Decoction low-dose group and model group were modeled as traumatic femoral head necrosis disease.After 4 weeks of modeling,one of the rats in each group was selected and their femoral head was extracted to verify the success.3.Drug intervention lasted for 6 weeks.Rats in the high,medium and low dose groups were given high,medium and low concentrations of Taohong Siwu Decoction,rats in the model group and the normal group were given equal doses of normal saline.The data were analyzed statistically.Results:1.General morphology observation: the femoral head of the rats in the model group was severely deformed and the surface cartilage was completely damaged;The shape of the femoral head in the three groups given taohong Siwu decoction was generally better than that in the model group,and the specimen in the high-dose taohong Siwu decoction group was close to that in the normal group.2.Histological examination:(1)Histological morphology: the sections of taohong Siwu Decoction high-dose group were close to those of the normal group,and the bone cells were abundant in quantity and good in quality,evenly distributed and compact,with full cell morphology and abundant collagen fibers.Collagen fibers were scarce in femoral head sections of model group.Taohong Siwu decoction medium dose group was between high dose group and model group.(2)The hollow bone fossa rate: taohong Siwu decoction low-dose group(52.55 ± 9.26),Taohong Siwu decoction medium-dose group(34.93 ± 7.72),Taohong Siwu decoction high-dose group(21.25 ± 5.89),model group(44.63 ± 4.50),normal group(21.38 ±5.6).There was no significant difference between taohong Siwu Decoction high-dose group and normal group(P > 0.05).There were differences among other groups in pairwise comparison(P < 0.05).3.Relative expression level of Osterix detected by RT-QPCR: The relative expression levels of Osterix in femoral head tissues of all groups were different(P < 0.05),and the expression levels from high to low were normal group,Taohong Siwu Decoction high-dose group,Taohong Siwu Decoction medium-dose group,Taohong Siwu Decoction low-dose group and model group,respectively,and there were differences in pial comparison(P <0.05).4.The relative expression level of Osterix detected by WB: the relative expression level of Osterix in femoral head tissue was different(P < 0.05),and the expression level from high to low was as follows: normal group,taohong Siwu Decoction high-dose,Taohong Siwu Decoction medium-dose,Taohong Siwu Decoction low-dose and model group;There was no significant difference between taohong Siwu Decoction medium-dose and high-dose groups and normal group in pair-wise comparison(P > 0.05).Conclusion:Taohong Siwu Decoction can up-regulate the expression of specific transcription factor Osterix in rats with traumatic femoral head necrosis,improve osteocyte activity of necrotic femoral head,repair its cartilage surface,prevent femoral head collapse and deformation,and enhance the stability of bone and hip joint;In the clinical treatment of traumatic femoral head necrosis,the dosage of Taohong Siwu Decoction should be higher than 80g(decocted and taken twice a day).
