Objective:Taking the patients with avascular necrosis of femoral head after total hip replacement as the research object, the clinical effect of Taohong Siwu Decoction on the prevention of constipation after total hip replacement based on the routine preventive measures after surgery was discussed, and a new idea was explored for the prevention of constipation after total hip replacement by traditional Chinese medicine. Methods:Sixty hospitalized patients who met the diagnostic criteria and inclusion criteria and underwent primary unilateral total hip replacement from December 2021 to December2022 were selected and randomly divided into control group and observation group according to random number table method.Control group: Routine preventive measures after total hip replacement;Observation group: on the basis of control group,Taohong Siwu Decoction was added for 7 consecutive days; There were 30 patients in each group. The time of the first postoperative autonomous defecation, the number of postoperative autonomous defecation, the score of abdominal distension and constipation symptoms, the number of defecation assisted by the use of unpacking gel and the efficacy evaluation of the two groups were observed. The collected data were compared within and between groups, and the relevant data was analyzed by SPSS25.0 software. Results:During the study, there was 1 case of shedding in the observation group because the patient voluntarily asked to quit, and 1 case of shedding in the control group because the patient failed to adhere to the medication.A total of 58 cases were completed, including 29 cases in the observation group and 29 cases in the control group.1.General information:Before treatment, there were no statistically significant differences between the two groups in age, gender,ASA score,BMI,ARCO stage of avascular necrosis of femoral head, surgical side, operation time and hemoglobin(P > 0.05).2.Scores of abdominal distension and constipation symptoms: there was no statistically significant difference between the observation group and the control group before surgery(P>0.05),but scores of abdominal distension and constipation in the observation group at 3days and 1 week after surgery were lower than those in the control group, the difference was statistically significant(P < 0.05).3.First postoperative autonomous defecation time: The observation group’s first postoperative autonomous defecation time was earlier than that of the control group, and the difference was statistically significant(P < 0.05).4.Postoperative spontaneous defecation times: The total number of defecation times 1 week after surgery in the observation group was higher than that in the control group, the difference was statistically significant(P < 0.05).5.The number of patients who used corkloo-assisted defecation: The number of patients who used corkloo-assisted defecation 1 week after surgery in the observation group was lower than that in the control group, and the difference between the two groups was statistically significant(P < 0.05).6.Comparison of efficacy: After 1 week of treatment, the effective rate of preventing constipation was 86.2% in the observation group and 62.1% in the control group, and the difference between the two groups was statistically significant(P < 0.05).Conclusions:After total hip replacement, taking peach red Siwu decoction on the basis of conventional treatment can effectively prevent the occurrence of constipation in patients, reduce the use of folium, restore the gastrointestinal function of patients, which is conducive to the early rehabilitation after hip replacement and improve clinical efficacy.
