Objective:To observe the clinical effect of manipulation based on the theory of five-element motions and six climatic changes in the treatment of knee osteoarthritis.Method: According to the inclusion criteria,62 inpatients and outpatients in the Department of Foot and Ankle Orthopedics of the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine were selected.The time period of the selected sick persons was from January 20,2022(the major cold solar term day of Xinchou Year)to January19,2023(the end of the minor cold solar term day of Renyin Year).The62 selected sick persons were divided into control and treatment groups by using the random number table method,and finally a total of 60 patients were collected,of which 30 were in the control and 30 in the treatment group,with a total of 2 shedding cases.The control group(hereinafter referred to as the general group)was conventional massage treatment(the "13th Five-Year Plan" coursebook of Tui Na for Higher Education in Chinese Medicine).The healing group was treated with manipulation under the guidance of the theory of five-element motions and six climatic changes(hereinafter referred to as Yun Qi manipulation group).The sick persons in both groups were treated for a total of 2 cycles,in which each patient completed 1 manipulation treatment per day,each lasting for about30 minutes,for a 7-day treatment cycle.Statistical analysis was performed on the data collected from the Yun Qi manipulation group and the conventional tui na group,respectively.Clinical outcomes were assessed using VAS,Lysholm and WOMAC.The proposed clinical efficacy assessment criteria are modelled on the principles in the Guiding Principles for Clinical Research on New Chinese Medicines(for Trial Implementation).The data of the clinically collected indicators were analysed using SAPSS24.0 statistical software.The differences in clinical outcomes between the two groups of sick persons after different cure modalities were objectively assessed based on the results of the analysis.Results:1.The five baseline data of age,weight,gender,disease duration and K-L imaging grade were statistically unpacked in the two groups.The results made known that there was non-noteworthy discrepancy between the Yun Qi manipulation group and the traditional massage group on the above five baseline data(p>0.05),allowing statistical comparison2.Comparison of overall clinical efficacy: In the 30 patients in the Yun Qi manipulation group,the overall effective rate was 93%,The number of sick persons who were controlled,significantly effective,effective and ineffective were 7,8,13 and 2,respectively..In the conventional massage group of 30 sick persons,the overall effective rate was 80%,the number of sick persons who were controlled,significantly effective,effective and ineffective were 2,3,19 and 6 respectively.The comparison was analyzed by chi-square test,and the result was 8.176,P=0.043(P <0.05),with a significant difference,indicating that the treatment of KOA based on the theory of five-element motions and six climatic changes combined with the rule of law manipulation along meridians was compared with the simple conventional manipulation,and the former was better in the treatment effect.3.VAS score: the comparison of the VAS scores of the two groups who underwent this clinical study before the start of Manual therapy suggested statistically that the difference was not noteworthy(P > 0.05),and that the VAS scores of both groups were significantly different(P < 0.05)after the completion of the two treatment cycles respectively,compared to the VAS scores before the start.This suggested that both treatments are clinically effective for KOA.The comparison between the two groups at the end of the manipulation displayed a significant difference between the groups(p < 0.05),making clearly that the Yun Qi manipulation group had a better remedy effect than the routine group.4.WOMAC scores: The total WOMAC score,pain,stiffness,and functional aspects of the established subgroups of sick persons were compared before the start of treatment,and the analysis yielded statistically insignificant differences(P > 0.05)in the four aspects of total score,pain score,stiffness score,and functional score.The four WOMAC scores of both groups after completing two cycles of manipulative therapy were significantly lower than the WOMAC scores before starting,with a significant difference(P < 0.05),which indicated that both treatments had good therapeutic effects on KOA.When the two groups of KOA patients were compared between groups after completing the treatment,the obtained P < 0.05 was significant,indicating that the treatment effect of the Yun Qi manipulation group was better than that of the routine group between the two means.Also,after comparing between groups in terms of pain,stiffness,and function,the P values obtained the conclusion that less than 0.05,suggesting that compared to the conventional massage group,the Yun Qi manipulation group could better eased KOA pain and stiffness,and better restore the joint function of KOA sick persons.5.Lysholm functional scores: when comparing the Lysholm function scores of the two groups of sick persons before the start of manipulation,there was non-significant difference between the two groups(P > 0.05),and after completion of two cycles of manipulation,the Lysholm scores of the two groups of sick persons were significantly higher than those before the beginning,with notable differences between the groups(P <0.05);that illustrates the good effect of treatment in both groups.When the scoring data of the two groups of sick persons were compared after the completion of the manual therapy,the derived P < 0.05,a notable difference,suggested that the Yun Qi group had a better result in restoring the function of the sick persons than the routine group.Conclusions: The manipulation based on the theory of five-element motions and six climatic changes has a good therapeutic effect on the pain caused by knee osteoarthritis,the influence of daily activities and joint stiffness,and the curative effect is better than that of conventional massage manipulation. |
