The Medication Rules And Clinical Observation Of Professor Song Liqun In The Treatment Of Cardiorenal Syndrome Ⅳ

Objective:Through the use of TMISS V3.0 to explore the application rules of Traditional Chinese Medicine in Professor Song Liqun’s treatment of chronic cardio-renal syndrome with Traditional Chinese Medicine,and further summarizes Dr.Song’s experience in the clinical treatment of this disease.and to conduct clinical observation based on this,and to enrich the clinical research of Chinese medicine treatment of chronic cardiac and renal syndrome of qi deficiency and blood stasis.Methods:Retrospective analysis part:Case data of patients in the expert outpatient service of Professor Song Liqun in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine were collected and analyzed by using TCMISS V3.0 to further explore the prescription medication rules of Professor Song in the treatment of CRS type Ⅳ.Clinical observation part:patients from the expert outpatient service of Professor Song Liqun from March 2022 to September 2022 were screened,40 patients who were eligible for CRS type Ⅳ Qi deficiency and blood stasis evidence were selected and given empirical prescriptions for treatment,The test was conducted for a total of 12 weeks to observe the patients’ cTn,or NT-proBNP,2D echocardiography and Doppler ultrasound,and Scr,BUN,CysC,and eGFR be-fore and after treatment.Results:Retrospective analysis:A total of 297 cases meeting the criteria are included.According to the statistical function of application frequency,192 drugs are used in the treatment of this disease,and the total drug frequency is 563 7 times.There are 62 traditional Chinese medicines used more than 20 times,accounting for 86.85%of the total frequency,with the highest frequency of use being "Atractylodis Macrocephalae-Radix Astragali",and the highest percentage of drugs used were warm drugs,sweet drugs and drugs classified as spleen meridian.We got 6 groups of core combination drugs,which are:Combination 1:Atractylodes Macrocephala,Radix Astragali,Apocyni Veneti Folium,Cooked rhubarb,Dandelion,Centella Asiatica,Clerodendranthus Spicatus,Eu com mi a Ulmoides Charcoal;Combination 2:Semen euryales,Astragalus Membranaceus,Radix Astragali,Ligustri Lucidi Fructus,Rosae Laevigatae Fructus Prescription,Poria cocos,Rubus chingii,Semen cuscutae;Combination 3:Astragalus Membranaceus,Radix Astragali,Ginkgo seed,Ligustri Lucidi Fructus,Semen euryales,Radix Puerariae,Endothelium corneum,Rubus chingii;Combination 4:Atractylodes macrocephala,Fructus trichosanthis,Radix Glycyrrhizae Preparate,Radix Astragali.Danshen,Shenqu,Malt,Hawthorn;Combination 5:Atractylodes Macrocephala,Astragalus,Shenqu,Malt,Hawthorn,Eucommia,Dulcimer,Eucommia charantia,Gorgonites;Combination 6:Atractylodes Macrocephala,Radix Astragali,Fructus Ligustri Lucidi,Rosa laevigata Michx,Semen eury ales,Radix curcumae,Caulis spatholobi,Poria cocos.Clinical observation part:the empirical prescription was more effective in treating patients with CRS type Ⅳ deficiency and blood stasis evidence,the total effective rate of Chinese medicine symptom complex score reached 92.5%,the total effective rate of chronic renal failure reached 80%,and the total effective rate of cardiac function reached 87.5%.Scr,BUN,Cys-C,NT-proBNP,cTn decreased after treatment compared with before treatment(all p<0.05).eGFR and EF%increased after treatment compared with those before treatment(all p<0.05),suggesting that this empirical prescription can invigorate the heart,spleen and kidney,promote blood circulation and removing blood stasis,therapy of nourishing healthy qi and eliminating blood stasis,help to strengthen the heart and kidney,improve the function and structure of both heart and kidney,and delay the development of the disease.Conclusion:1.In the treatment of CRS type Ⅳ by Professor Song Liqun,the most frequently used prescription is "Atractylodis Macrocephalae-Radix Astragal",with the highest proportion of warming drugs,sweet-tasting drugs and drugs classified as spleen meridians.2.Atractylodis Macrocephalae,Radix Astragalal,Fructus Ligustri Lucidi,semen euryales,Rosa laevigata,Poria cocos,Ginkgo seed,Pericarpium Citri Reticulatae,Yujin,Salvia miltiorrhiza,Hawthorn,Rub us chingii,Shenqu,Malt,Apocynum venetum L,Cooked rhubarb,Fructus trichosanthis,Caulis spatholobi,Dandelion,Smilax glabra Roxb.these drugs are the core drugs used by Pro fessor Song Liqun in the treatment of CRS.Astragali Radix-A tractylodis Macrocephalae Rhizoma is the most commonly used drug pair by Professor Song Liqun in the treatment of this dise ase.The compatibility relationship between core drugs is very s table and has strong regularity.3.Professor Song Liqun believes that CRS type Ⅳ is a kind of syndrome that root vacuity and tip repletion.The deficiency of the heart,spleen and kidney;In fact,dampness,heat,blood stasis and poison are the main problems.4.This empirical prescription can reduce the TCM clinical symptoms of patients with CRS type Ⅳ Qi deficiency and blood stasis,lower Scr,BUN,CysC,NT-proBNP and cTn,effectively protect the functions of the heart and kidney,and delay the progression of CRS type Ⅳ.5.This prescription can improve eGFR and EF in patients with CRS type Ⅳ with Qi deficiency and blood stasis,effectively protect the function of both heart and kidney,and delay the progression of CRS type Ⅳ.
