Objective:Through the combination of theoretical research and clinical and epidemiological investigation,this topic studies the etiology and pathogenesis of pulmonary nodules,explores its clinical pathogenesis,and discusses the role of liver and spleen in the occurrence and development of pulmonary nodules,providing new ideas for clinical diagnosis and treatment of pulmonary nodules.Methods:This study adopts the methods of theoretical research and clinical investigation.In theory,we mainly study the physiological and pathological relationship between liver,spleen and lung by combing the ancient and modern data,so as to explore the relevant content of liver,spleen and lung nodules.The clinical research is mainly based on the current situation survey.According to the inclusion and exclusion criteria,173 patients with pulmonary nodules were selected.The clinical data were collected in the form of questionnaires,and the TCM syndrome type,physical classification and HAD anxiety/depression tendency were determined.The clinical data were sorted and recorded in Excel,and the clinical data database of pulmonary nodules was established and analyzed using SPSS 27.0 software,in order to explore the general situation of patients with pulmonary nodules TCM syndrome type,TCM constitution,HAD anxiety/depression tendency and other kinds of distributions to analyze the general incidence law of pulmonary nodules.Combined with the theoretical knowledge of traditional Chinese medicine,the statistical results are discussed and the conclusion is drawn.Combined with the is found that pulmonary sarcoidosis is located in the lung,closely related to the dysfunction of the liver and spleen.Liver-qi stagnation and deficiency of spleenare particularly critical in the pathogenesis of the disease.Therefore,the treatment of pulmonary nodules should be based on the liver and spleen,focusing on soothing the liver and relieving depression,invigorating the spleen and replenishing qi,as well as resolving phlegm and stasis.2.Clinical study:The study of 173 patients with pulmonary nodules found that 59 of them were males while 114 were females,females were significantly more than males.Among those patients,147 are middle-aged and elderly patients aging from 41 to 65 years old while ones over 66 years old,accounting for 84.97%.From the perspective of occupational distribution,42.77%of the patients are retirees.36.42%of the patients smoked,including passive second-hand smoke.Among 173 patients,30.64%had chronic lung disease and 20.81%had family tumor history.72.83%of the patients were accompanied with thyroid nodules,breast hyperplasia or other diseases.Meanwhile,64 were solitary nodules and 109 were multiple nodules.Among the patients with solitary nodules,there were 42 solid nodules,20 pure ground-glass nodules and 2 partial solid nodules.As for the patients with multiple nodules,there were 58 solid nodules,26 pure ground-glass nodules,4 partial solid nodules,19 solid nodules with pure ground-glass nodules,and 2 pure ground-glass nodules with partial solid nodules;In addition,168 patients did not have symptoms such as lobulation or burr.Patients with pulmonary nodules often have irritability,anxiety and other adverse emotions,and clinical symptoms such as fatigue and weakness,irritability,anxiety and thinking,shortness of breath,chest tightness,dry mouth,foreign body sensation in the throat,chills,cough turbidity and tightness,thick expectoration,less expectoration,insomnia,loss of appetite,bitter mouth,fullness,spontaneous sweating,dizziness,dry throat,loose stool,good breath,pulse string and other symptoms that are closely related to the dysfunction of the liver and spleen.From the distribution of TCM syndrome types,the frequency of liver depression and spleen deficiency syndrome,and qi stagnation and phlegm obstruction syndrome is relatively high,followed by lung and spleen deficiency syndrome,phlegm and blood stasis syndrome,yang deficiency and qi depression syndrome,lung qi and yin deficiency syndrome,and other syndrome types.The Traditional Chinese Medicine constitution of the patients is mostly biased and one has two or more kinds of constitution.The first constitution is dominated by qi deficiency constitution constitution,yang deficiency constitution and qi stagnation constitution,while the second constitution is dominated by qi stagnation constitution,qi deficiency constitution and phlegmdampness constitution.Conclusion:1.Liver qi stagnation and spleen deficiency are the core pathogenesis of pulmonary nodules.Depression and inappropriate diet lead to liver-qi stagnation,deficiency of spleen,stagnation of qi and body fluid,obstruction of blood flow,resulting in stagnation of qi,phlegm-dampness,static blood,which can finally grows to pulmonary nodules.2.Clinical research shows that the clinical symptoms of pulmonary nodules are closely related to the dysfunction of the liver and spleen.Therefore,lung nodules can be treated from the liver and spleen,and the liver and spleen should be in tune during the treatment,such as strengthening the spleen and replenishing qi on the basis of soothing the liver and relieving depression,in order to resolving the phlegm and blood stasis.3.Most patients with pulmonary nodules have biased constitution,such as qi stagnation constitution,qi deficiency constitution,Yang deficiency constitution and phlegm-dampness constitution.
