Objective:Observe the clinical effect of acupuncture treatment of benign thyroid nodules(hepatic stagnation and phlegm coagulation type)with "Shugan Jianpi method",and explore the mechanism of acupuncture treatment of thyroid nodules.Method:According to the inclusion criteria,74 subjects were enrolled,and they were divided into 2 groups by random number table,37 in the control group and 37 in the treatment group,and the course of treatment was 4 weeks.The control group took reasonable work and rest,avoiding long-term work,proper exercise,psychological counseling,soothing emotions,and abstaining from bad habits(tobacco,alcohol,etc.)and other life intervention methods.The treatment group used acupuncture techniques on the basis of the control group to select acupuncture points: Tianshu,Zusanli,Sanyinjiao,Taichong,Xingjian,Qimen,Baihui,Neiguan.The acupuncture points were operated according to the method of replenishing and reducing,the needles were retained for 30 minutes,and the acupuncture was performed once every 10 minutes.3 times a week,6 times as a course of treatment,a total of 2courses of treatment.Result:1.After the end of the trial,the TCM syndrome scores of the two groups were reduced compared to the time when they were included(P<0.05),and the effective rates of the two groups were 50% and 91.42%,respectively.The difference was statistically significant(P<0.05);In terms of remission,after the intervention,the two groups had statistically significant differences in throat discomfort,chest tightness,excessive phlegm,insomnia,stool disorders,and irregular menstruation(P<0.05).2.Comparing the scores of anxiety and depression before and after treatment in the two groups,the difference was statistically significant(P<0.05).After treatment,the scores of anxiety and depression between the groups were compared,(P<0.05),the difference was statistically significant.3.The difference in the maximum diameter and volume of thyroid nodules within the two groups before and after treatment and between the groups was statistically significant(P<0.05),and the difference in the number of thyroid nodules was not statistically significant(P<0.05);After 4 weeks of treatment,the effective rates of nodule diameter improvement in the two groups were 91.42% and 44.44%,respectively,and the difference was statistically significant(P<0.05).4.The thyroid function of the two groups was within the normal range before and after treatment.There was no statistically significant difference in the comparison between the groups before and after the treatment and the comparison between the groups after intervention(P>0.05).5.After the test,the difference in insulin resistance index between the two groups before and after the intervention and after the intervention was statistically significant(P<0.05).Conclusion:1.Under the guidance of the theory of treating thyroid nodules from the liver and spleen,acupuncture for soothing the liver and strengthening the spleen is effective in treating benign thyroid nodules.2.Compared with lifestyle intervention,the soothing liver and spleen acupuncture method has certain advantages in alleviating TCM symptoms,regulating mood,delaying the size of nodules,and improving insulin resistance.3.Soothing the liver and strengthening the spleen acupuncture treatment of thyroid nodules may delay the progress of nodule size by improving insulin resistance,improving bad mood,and correcting metabolic disorders.
