| Objective: Through the application of Huoxue Zhentong decoction in the auxiliary treatment of early postoperative pain in patients with calcaneal fracture,the changes of inflammatory mediators related to pain were observed,and the clinical efficacy of Huoxue Huayu prescription in the treatment of early postoperative pain was evaluated.Methods: In this study,62 patients with calcaneal fracture who met the inclusion criteria but not excluded criteria in the Department of Orthopaedics of Fuzhou second Hospital from January 2021 to February 2022 were randomly divided into experimental group(n=31)and control group(n=31).62 patients with calcaneal fracture were treated with general anesthesia and sciatic nerve block anesthesia by the same group of anesthesiologists.The same group of doctors were treated with open reduction and plate internal fixation of calcaneal fracture through lateral L-shaped approach.The control group was given intravenous flurbiprofen axetil injection for pain relief and routine treatment 5 days after operation,while the experimental group was given Huoxue analgesic decoction on the basis of the control group(twice a day for 5 days).On the 1st,3rd and 6th day after operation,fasting venous blood was taken to detect the concentration of serum interleukin-6(IL-6),bradykinin(BK)and substance P(SP)by enzyme-linked immunosorbent assay(ELISA).The visual analogue scale(VAS)of pain on the 1st,3rd and 6th day after operation was observed and recorded,and the analgesia satisfaction of the two groups was recorded on the 6th day after operation.The values of glutamic pyruvic transaminase,glutamic oxaloacetic transaminase and creatinine were collected on the 1st day and 6th day after operation.Results: The experimental group and the control group lost 1 case respectively,and finally completed the clinical trial study in 60 cases,30 cases in the experimental group and30 cases in the control group.1.General data: there was no significant difference in sex,age,operative side and Sanders classification between the two groups(P<0.05).2.Postoperative VAS score: comparison between groups: there was no significant difference in VAS score between the two groups on the first day after operation(P>0.05).But the VAS score of the experimental group was lower than that of the control group on the 3rd day and 6th day after operation(P<0.05).Intra-group comparison: the VAS score of the two groups decreased gradually on the 1st,3rd and 6th day after operation,and the difference was statistically significant(P<0.05).3.Serum inflammatory mediators: there was no significant difference in IL-6,BK and SP between the two groups on the first day after operation(P>0.05).But the values of IL-6,BK and SP in the test group were significantly lower than those in the control group on the 3rd and 6th day after operation(P<0.05).Intra-group comparison: it was lower on the 3rd day after operation than that on the 1st day and the 6th day after operation in the two groups,and the difference was statistically significant(P<0.05).4.Analgesia satisfaction(Likert score): the test group was completely satisfied with36.67%,relatively satisfactory 43.33%,general 20%;the control group was completely satisfied with 16.67%,relatively satisfactory 60%,general 23.33%;there was no patient dissatisfaction in both groups,and there was no significant difference in Likert score between the two groups(P<0.05).5.Safety index: there was no significant difference in glutamic pyruvic transaminase,glutamic oxaloacetic transaminase and creatinine between the two groups on the 1st day and6 th day after operation(P<0.05).There was no significant difference in glutamic pyruvic transaminase,glutamic oxaloacetic transaminase and creatinine between the two groups on the 1st day and 6th day after operation(P<0.05).Conclusion: Huoxue Zhentong decoction can reduce the concentration of serum IL-6,BK and SP in patients with calcaneal fracture,and effectively relieve early postoperative pain in patients with calcaneal fracture. |
