| Objective:1.To observe the clinical efficacy of clearing damp Jiedu Huoxue decoction combined with Shengji Yuyang cream in treatment of ecthyma(dampness-heat stasis type).2.Dynamic monitoring before and after treatment by 14 d and 28 d after treatment serum interleukin-1(Intelleukin-1,IL-1),interleukin-6(Intel leukin-6,IL-6)levels,explore the clearing damp jiedu huoxue soup joint production of the preventive treatment cream ecthyma(dampness heat and blood stasis resistance)of the possible mechanism of action,for clinical use of TCM prevention and treatment of ecthyma provides some theory reference.Methods: Sixty patients with ecthyma from the TCM surgery department of the affiliated hospital of Jiangxi University of Chinese Medicine were collected and randomly divided into treatment group and control group.The treatment group was given internal administration of clearing damp Jiedu Huoxue Decoction combined with external use of Shengji Yuyang Ointment.The control group was given calcium hydroxybenzene sulfonate capsule orally combined with silver sulfadiazine cream for external use.The treatment lasted for 14 days as one course,and the observation period was 2 courses.The wound area,serum IL-1 and IL-6 levels of the patients were recorded before,14 and 28 days after treatment.The ulcer wound healing rate and ulcer healing rate after 14 and 28 days of treatment were recorded.Specific wound symptom score(The granulation of the wound 、 the smell of the wound 、 the oozing pus 、 the pain of the wound)and total wound symptom score were recorded before and after treatment,and these observation indexes were analyzed and compared.Results:1.The total effective rate of patients in the two groups was compared.The treatment group was 90% higher than the control group(83.33%),and the effective rate in the treatment group was significantly better than the control group(P<0.01).2.Ulcerative wound area and IL-1 and IL-6 levels of patients in both groups were analyzed between and within groups after 14 d and 28 d of treatment.It was found that the ulcerative wound area,IL-1 and IL-6 levels in both groups were improved after 14 d and 28 d of treatment,and the treatment group was superior to the control group(P<0.05).3.The wound healing rate and wound healing speed of the two groups were compared after 14 d and 28 d of treatment,and the treatment group was better than the control group(P<0.05).4.the two groups of patients with specific wound integral(wound granulation,odor,seeps the pus,the pain of the wound)and total score after treatment group and the contrast between groups,two groups of patients with specific wound integral(granulation,the smell of the wound,wound pain)and total integral wound after treatment was lower,the superior to that in the control group and treatment group(P<0.05).After treatment,the score of pus exudate in both groups was lower than before,while the score of pus in the treatment group was higher than that in the control group.Conclusion:1.Clearing damp Jiedu Huoxue Decoction combined with Shengji Yuyang Paste has a positive effect on the treatment of ecthyma(dampness and heat stasis type),and the effect is more significant than that of calcium oxybensulfonate capsule combined with silver sulfamidine.It reflects the advantages and characteristics of traditional Chinese medicine in the treatment of lower extremity chronic venous ulcer,and the program is worthy of clinical application and promotion.2.By observing the levels of IL-1 and IL-6 in serum before and after treatment,it was concluded that the combination of clearing damp Jiedu Huoxue Decoction and Shengji Yuyang Ointment promoted wound healing,which may be related to the regulation of serum IL-1and IL-6 content.However,the specific mechanism still needs to be further studied. |
