The Clinical Study On The Regulation Of Inflammatory Response By Detoxifying And Eliminating Stasis Chinese Herb To Promote The Damp-Heat Type Ecthyma

Objective: To investigate the clinical efficacy of detoxifying and eliminating stasis Chinese herb in promoting the damp-heat type ecthyma through regulating the inflammatory response based on the AIM2/Caspase-1/GSDMD pyroptosis pathway,and to explore its mechanism of action.Methods: Seventy-five patients with damp-heat type ecthyma were collected and divided into three groups using the random number table grouping method: Gong Ying detoxification lotion group,Fufang Huangbai Liquid group and blank control group.25 patients in each group,75 patients were treated with the same basic treatment regimen + different external treatment regimens,with the Gong Ying Detoxifying Lotion group applying Gong Ying Detoxifying Lotion externally to the wounds,the Fufang Huangbai Liquid group applying Fufang Huangbai Liquid application externally to the wounds and the blank control group leaving the wounds untreated.The biological indexes,the TCM syndrome score of the trauma and peri-trauma,and the healing rate of the trauma were observed on days 1,7 and 14 of treatment in the three groups,and the expression levels of AIM2,Caspase-1 and GSDMD mRNA in the peri-trauma venous blood of the three groups were detected by RT-PCR.Results:1.General reference: A total of 75 cases were enrolled in this study,and there were no shedding cases in the three groups during treatment.Statistical analysis of gender,age,site of onset,and trauma area of the three groups showed no statistical differences(P > 0.05),and the baseline data of the three groups were consistent and could be compared for clinical efficacy observation.2.Clinical efficacy:(1)By the overall efficacy analysis,the Gong Ying Detoxifying Lotion group was superior to the Fufang Huangbai Liquid group than the blank control group,with statistical difference(P<0.05).(2)On the 7th day of treatment,the WBC and CRP values of the Gong Ying Detoxifying Lotion group and the Fufang Huangbai Liquid group were statistically different from those of the blank control group(P<0.05),and the WBC and CRP values of the Gong Ying Detoxifying Lotion group and the Fufang Huangbai Liquid group were no statistically different(P>0.05).The Gong Ying Detoxifying Lotion group and the Fufang Huangbai Liquid group were statistically superior to the blank control group in improving the wound healing rate(P<0.05).The Gong Ying Detoxifying Lotion group was better than the Fufang Huangbai Liquid group and the blank control group in improving the wound surface and the periwound syndromes score,with statistical differences(P<0.05).(3)On the 14 th day of treatment,the WBC and CRP values of the Gong Ying Detoxifying Lotion group and the Fufang Huangbai Liquid group were statistically different from those of the blank control group(P<0.05).The Gong Ying Detoxifying Lotion group and the Fufang Huangbai Liquid were better than the blank control group in improving the wound healing rate,wound surface and peri wound syndromes score,with statistical differences(P<0.05).3.For RT-PCR indicators:(1)The overall trend of AIM-2,Caspase-1 and GSDMD mRNA expression levels in the Gong Ying Detoxifying Lotion and Fufang Huangbai Liquid groups were higher at day 7 and lower by day 14,and higher at day 7 in the blank control group,although lower at day 14,but still at a higher level.(2)On the 7th and 14 th day of treatment,the expression levels of AIM-2,Caspase-1 and GSDMD mRNA in the Gong Ying Detoxifying Lotion and Fufang Huangbai Liquid groups were statistically different from those in the blank control group(P<0.05).There was no statistical difference(P>0.05)between the Gong Ying Detoxifying Lotion group and the Fufang Huangbai Liquid group.(3)The expression levels of AIM-2,Caspase-1 and GSDMD mRNA decreased on day 14 in the Gong Ying Detoxifying Lotion group and the Fufang Huangbai Liquid group compared with day 7,with statistical differences(P<0.05).Conclusion:1.Gong Ying Detoxifying Lotion can effectively promote the healing of damp-heat type ecthyma.2.The dynamic monitoring of WBC,CRP,wound surface and periwound syndromes score and healing rate statistics showed that Gong Ying Detoxifying Lotion has strong anti-inflammatory effect.Gong Ying detoxifying lotion and Fufang Huangbai Liquid can effectively reduce AIM-2,Caspase-1 and GSDMD mRNA expression in patients with damp-heat type ecthyma,where the healing process involves traumatic pyroptosis.
