Objective:To construct the core competence evaluation system for postgraduates with Master of nursing specialist and clinical nurse specialist, and to provide a reference for conforming the joint training program of Master of nursing specialist and clinical nurse specialist, including training objective, the theory of curriculum, the program of clinical practice, evaluation mode and assessment indicator system. This study means to provide a reference for the development of nursing graduate education and the improvement of the postgraduate education level.Methods:This study is divided into two parts. The first part is to determine the core competence evaluation system for postgraduates of Master of Nursing specialist and clinical nurse specialist. On the basis of literature review and group discussion, we construct the core competence evaluation system for postgraduates of Master of Nursing specialist and clinical nurse specialist preliminarily. According to the system, we designed "Postgraduate students of Master of Nursing specialist core competences expert advisory questionnaire"and "Clinical nurse specialist core competences expert advisory questionnaire", all the indexes are screened by Delphi expert consultation method to determine the core competence evaluation system for postgraduates of Master of Nursing specialist and clinical nurse specialist ultimately. The second part is to determine the joint training program of Master of nursing specialist and clinical nurse specialist, including training objective, the theory of curriculum, the program of clinical practice, evaluation mode and assessment indicator. On the basis of the core competence evaluation system of postgraduates of Master of Nursing specialist and clinical nurse specialist, we construct the composition of training objective, the theory of curriculum, the program of clinical practice, evaluation mode and the core competence of assessment indicator system. All the indexes are screened by Delphi expert consultation method. We sent E-mail, mail or on-site payment to consult the experts. When the result of statistical analysis indicated that the expert opinions tend to be consistent, the rounds of consultation is over.Results:Experts'opinions tend to be consistent after two rounds of consultation. The effective response rate of the two rounds of consultation were 91.67%and 93.93%; the expert authority coefficients were 0.825,0.88; the coefficient of variation of first-grade indexes?second-grade indexes and the third-grade indexes of MNS were 0.109?0.170?0.093?0.172?0.097?0.163; coordination coefficient were 0.29. 0.23 and 0.25 (P<0.01). The coefficient of variation of first-grade indexes% second-grade indexes of CNS were 0.101-0.131?0.059?0.172?0.064-0.175; coordination coefficient were 0.27?0.22 and 0.24 (P<0.01). The core competence evaluation system for postgraduates of Master of Nursing specialist was composed of 8 first-grade indexes (Ideological and political quality and professional moral accomplishment, Theory knowledge, Clinical nursing practice ability, Nursing management ability, Nursing education ability, Scientific research ability, Critical thinking ability, Humanistic quality),18 second-grade indexes and 38 third-grade indexes. The core competence evaluation system for clinical nurse specialist was composed of 8 first-grade indexes (Ideological and political quality and professional moral accomplishment, Specialized nursing theory knowledge, Specialized nursing practice skills, Nursing management ability, Nursing education ability, Scientific research ability, Clinical analysis and thinking ability, Professional development ability),18 second-grade indexes and 39 third-grade indexes. The experts' opinions tend to be consistent after two rounds of consultation in the second part of the study, too. The effective response rate of the two rounds of consultation were 91.67% and 90.91%; the expert authority coefficients were 0.825,0.875; the coefficient of variation was 0.041?0.227; coordination coefficient were 0.26 (P<0.01). The training objectives include:Having a good ideological and political quality and professional moral accomplishment, mastering a solid nursing basic theory and professional knowledge of the system, grasping skilled professional practice skills, being able to solve the problem of common nursing specialist areas and specialized problems independently, being able to use the nursing process, being able to get engaged in nursing education independently, having high ability of clinical scientific research and innovation, with strong communication and team cooperation ability, as well as the specialized development ability. The theory of curriculum include:Public degree course, Professional required degree courses, Professional elective courses and Academic activities. Public degree course include English of degree and professional foreign language; Professional required degree courses include Clinical epidemiology, Medical statistics, Medical research methodology, New progress in clinical care and nursing management, The specialized nursing theory and practice and Evidence-based nursing theory and method; Professional elective courses include Senior health assessment, Critical thinking and clinical decision making, Care and clinical diagnosis and treatment technology progress, Interpersonal communication and exchanges. Academic activities include participating in humanities and scientific literacy lecture series at school and hospitals for at least 8 times, participating in academic activities for at least 2 times, writing at least 1 scientific paper. The program of clinical practice can be summarized as this: clinical practice begins in the second semester of the first year for 30 months, Department of clinical practice include department accords with Professional research direction, Internal medicine department (neurology, endocrinology, cardiology), Surgical department (general surgery, orthopedics, chest cardiac surgery), ICU and emergency department. Practice contents include Specialized nursing technical operation training, Shifts in clinical nursing work, being responsible of case study of patients with specialized diseases, participating in collaborative nursing specialist team, participating in the department specialized nursing ward round and In typical department specialized cases discussions, Participation in clinical teaching and the consultation specialist patient care. The evaluation mode include:Clinical skills examination, case discussion, Case teaching and nursing ward round, giving a lesson, case study, Team collaborative nursing care, academic dissertation, scientific research innovation and comprehensive quality. The assessment indicator system is composed of 9 first-grade indexes(The ideological and political quality and professional moral accomplishment, theoretical knowledge, clinical practical skill, Nursing education ability, Clinical scientific research ability, Clinical analysis and thinking ability, Professional development ability, humanistic quality and psychological quality) and 22 second-grade indexesConclusions:1. The core competence evaluation system for postgraduates of Master of Nursing specialist was composed of Ideological and political quality and professional moral accomplishment, Theory knowledge, Clinical nursing practice ability, Nursing management ability, Nursing education ability, Scientific research ability, Critical thinking ability, Humanistic quality. Clinical nursing practice ability is the main core competence of postgraduates of Master of Nursing specialist.2. The core competence evaluation system for clinical nurse specialist was composed of Ideological and political quality and professional moral accomplishment, Specialized nursing theory knowledge, Specialized nursing practice skills, Nursing management ability, Nursing education ability, Scientific research ability, Clinical analysis and thinking ability, Professional development ability, Specialized nursing practice skills is the main core competence of postgraduates of Master of Nursing specialist.3. The joint training program of Master of nursing specialist and clinical nurse specialist based on The core competence evaluation system for postgraduates of MNS and CNS is beneficial to improve the comprehensive quality of mater of nursing specialist students, which can cultivate a group of specialists and high level experts to adapt to the needs of the development of the society and economy. The program will promote the long-term development of nursing postgraduate education and to drive the specialization process.
