Construction Of Evaluation Index System Of ICU Nurses' Core Competence And Preliminary Application

ObjectiveIn this study,Delphi method was used to build a set of scientific and reasonable evaluation index system of ICU nurses core competence,which is in line with China's national conditions.At the same time,the reliability and validity of the evaluation index system of ICU nurses'core competence were tested to verify its effectiveness and feasibility,which provides a reference for the evaluation and assessment of ICU nurses' core competence in China,and is helpful to the refinement,specialization and specialization of ICU nurses The implementation of the target;to provide reference for the curriculum of critical care education;to promote the development of critical care in China.Methods:1.Through literature review and theoretical analysis,with reference to relevant scales and guidelines,combined with the national nursing development plan(2016-2020)and the development status of ICU nurses in China,we preliminarily constructed an item pool of core competence indicators of ICU nurses.2.Combining Delphi method and AHP,using Yaahp 10.1 to determine the weight coefficient of the evaluation index system of ICU nurses' core competence.The reliability of this study was analyzed by using the positive coefficient of experts,the authoritative coefficient of experts and the Kendall's w coordination coefficient of experts' opinions.The mean value,the coefficient of variation and the full score ratio of the items were used to select and modify the items,and finally the evaluation index system of the core competence of ICU nurses was established.3.The constructed evaluation index system of ICU nurses was applied initially,and the ICU nurses meeting the acceptance and discharge standards were investigated.The data were analyzed by extreme value test and correlation coefficient(correlation coefficient between each item and total score and its secondary index),and the items were further screened by Cronbach's The reliability of a coefficient was tested,and the content validity and structure validity were used to test the validity.Results:1.The evaluation index system of ICU nurses' core competence constructed in this study includes 5 first level indexes,20 second level indexes and 74 third level indexes.2.The authority coefficient of expert consultation is 0.829,and the coordination coefficient of two rounds of expert consultation is 0.157-0.229.The difference is statistically significant by chi square test(P<0.05).3.The results of analytic hierarchy process showed that:the first level index weight scores from high to low were professional clinical practice ability,clinical judgment and thinking ability,professional basic theoretical knowledge,professional development ability and nursing management ability,with weights of 0.278,0.244,0.186,0.184 and 0.108,respectively;the second level index weight scores showed that the top 5 were rescue cooperation ability,clinical insight ability and independence The weights of learning ability,emergency process knowledge and self reflection ability are 0.129,0.090,0.075,0.072 and 0.069 respectively;the top five scores of the three-level index weight coefficient are:Mastering the ICU nursing core system,job responsibilities,work processes and related emergency plans;dynamically mastering the patient's condition changes,and responding to emergencies quickly;correctly assessing the unstable and potential threat students of patients Life factors;be able to foresee and evaluate the possible nursing risks of patients;correctly judge cardiac arrest and implement rescue measures such as cardiopulmonary resuscitation technology,with weights of 0.053,0.041,0.029,0.026 and 0.025 respectively.4.The evaluation index system of ICU nurses' core competence showed that:the T value of each index was between 7.521 and 15.754(T>3,P<0.001);the correlation coefficient between each index score and the total score was 0.596-0.907(P Construct validity.Among them,the total Cronbach's a coefficient is 0.993,the Cronbach's a coefficient of each primary index is 0.965-0.983,the CVI value of content validity is 0.818-1.000,the kmo value of structure validity is 0.949,the ballett spherical test value is 17712.260(P<0.01),and the cumulative variance contribution rate is 79.62%.5.The core competence score of ICU nurses was(268.61±50.55).The results of single factor analysis showed that there were significant differences in age,title,job satisfaction and working years of ICU(P<0.05).Conclusions:1.Combining Delphi method and analytic hierarchy process,this study constructs the evaluation index system framework of ICU nurses' core competence,including 5 first level indexes,20 second level indexes and 74 third level indexes,and establishes the corresponding weight coefficients for each level indexes.The expert opinions are concentrated,the content is comprehensive and the structure is reasonable.2.The reliability and validity of the evaluation index system of ICU nurses' core competence are good,which can provide scientific reference for the evaluation and cultivation of ICU nurses' core competence,and also provide certain guidance for the development of ICU nurses themselves.3.This study found that the core competence of ICU nurses is in the middle level,and should focus on the ICU nurses with small age,low professional title,low job satisfaction and long working years,so as to take targeted interventions to improve the core competence of ICU nurses.
