ObjectivesObjective the purpose of this study is to clarify the constituent elements and specific connotation of the core competence of new internal medicine nurses in Grade 3A General Hospital by Delphi method.based on the theoretical framework of core competence,the evaluation index system of core competence of new nurses in internal medicine was preliminarily constructed and the index weight was determined,so as to provide a scientific and objective evaluation tool for nursing managers to define and assess the core competence of new nurses in internal medicine.It also points out the direction for the next step of cultivating the core competence of new nurses in internal medicine.MethodsThe theoretical framework was screened by the method of literature analysis,the present situation of the research on the evaluation of the core competence of new internal medicine nurses in Grade 3A General Hospital was reviewed,the initial indicators were collected,and through expert meetings and research group discussions,the primary draft of the evaluation index system for the core competence of new internal medicine nurses in Grade 3A Comprehensive Hospital was drawn up,and the Delphi expert letter method was adopted.Based on the mobile platform of "questionnaire Star+Wechat",20 domestic internal medicine nursing experts were inquired by correspondence for two rounds,and the index weight was determined and ranked by AHP analytic hierarchy process,thus forming the final evaluation index system of core competence of new internal medicine nurses in Grade 3A General HospitalFindings1.In terms of the enthusiasm of experts:20 questionnaires were sent out in the first round of letters,20 were recovered,with an effective recovery rate of 100%.In the second round,20 questionnaires were sent out and 18 were recovered,with an effective recovery rate of 90%.n the two rounds of consultation,10 and 3 experts each put forward Suggestions for revision,indicating that the experts were highly motivated2.In terms of the degree of expert authority:in the first round,the judgment basis of experts is 0.875,the degree of familiarity is 0.770,and the coefficient of authority is 0.823.In the second round,the basis of expert judgment is 0.883,the degree of familiarity is 0.778,and the coefficient of authority is 0.835.The two rounds of letter inquiry expert authority coefficient(Cr),are greater than 0.7,indicating that the degree of expert authority is high3.In terms of the degree of expert coordination,the Kendall harmony coefficients of the first,second and third?level indicators are 0.339,0.200 and 0.196 respectively,and the Kendall harmony coefficient of the overall index is 0.202.After the expert coordination degree was tested by ? 2 test,the P values were all less than 0.01,indicating that the expert score tended to be consistent,which was statistically significant.4.The core competence index system of new nurses in internal medicine department of Grade 3A General Hospital:after two rounds of letter inquiry,the average value of the first?level index was 4.06?4.94,the coefficient of variation was 0.05?0.22,the average value of the second?level index was 4.11?4.94,the coefficient of variation was 0.05?0.18,the average value of the third?level index was 4.22?5.00,and the coefficient of variation was 0.00?0.19.The evaluation index system of core competence of new nurses in internal medicine department of Grade 3A General Hospital is formed,which includes 6 first?level indicators,23 second?level indicators and 53 third?level indicators.The first?level indicators are professional accomplishment,theoretical knowledge,clinical practice ability,clinical management ability,communication and education ability,and career development The secondary indicators are physical and mental quality,professional cognition,professional attitude,practice according to law,basic knowledge of medicine/nursing,specialist nursing skills,emergency/first aid skills,critical thinking,records of nursing documents,use and management of instruments and equipment,time management,quality management,cost management,coordination ability,environmental safety,chronic disease management,communication among colleagues,Communication with leaders,communication and education of caregivers or guardians,continuing education,self?presentation5.Weight construction of evaluation index system:through AHP analytic hierarchy process,the weights of six first?level indicators are 0.333,0.137,0.229,0.044,0.182,0.076 respectively;the combined weight of secondary indicators is 0.003?0.139;through the proportional distribution method,the combined weight of three?level indicators is 0.001?0.073.The CR value of each judgment matrix is less than 0.1ConclusionsBased on the theoretical framework of core competence of registered nurses in China,the evaluation index system of core competence of new internal medicine nurses in Grade 3A General Hospital was constructed in this study.Based on literature analysis and discussion of the research group,the primary framework of evaluation index system was drafted,and expert pre-investigation was conducted.Through two rounds of multi-center expert letter inquiries and tests,the ultimate evaluation index system of core competence of new internal medicine nurses in Grade 3 A General Hospital was determined.The construction process is rigorous and scientific,and the results are true and reliable.At the same time,this study uses the AHP method to objectively show the index weight of all levels of the index system,which provides an authoritative evaluation tool for the standardized training and comprehensive evaluation of new internal medicine nursing nurses in our country.
