Preliminary Construction Of Nursing Quality Evaluation Index System Of Intensive Care Unit Patients' Physical Restraint

ObjectivesThe purpose of this study is to establish an scientific index system to evaluate the quality of Intensive Care Unit restraint nursing care through literature review,theoretical analysis,qualitative interviews,Delphi method expert consultation and Analytic Hierarchy Process,to compare the ICU patient physical restraint horizontally or vertically for nursing managers provides reference basis for restricting nursing quality.MethodsThrough literature review and theoretical analysis,combined with the status quo of physical restraint nursing quality of domestic ICU patients,the study was based on the "structure-process-result" model as the theoretical basis,preliminary screening of ICU patients' physical restraint nursing quality evaluation indicators.Semi-structured interview method was used to improve the index content;Delphi method was used to conduct two rounds of questionnaire consultation with 26 experts from 18 provinces and cities to determine the evaluation index system of ICU patients' physical restraint nursing quality.Finally,AHP was adopted to calculate the weight of each index.Check its logical consistency.Results1.By using the "structure-process-result" theory as the theoretical framework,an evaluation index system for the ICU nursing quality of physical restraint is established,which is divided into structure quality,process quality and result quality,including 3 first-level indicators and 10 second-level indicators,42 three-level indicators.The positive coefficients of the two rounds of expert consultations are 96.3% and 100%,respectively,indicating that the experts are active in this research;the expert's authoritative coefficients are both 0.96,indicating that the experts have a higher degree of authority;The coordination coefficients of the two rounds of expert consultations were 0.246 and 0.205(p <0.001),the conclusion was reliable.In summary,expert opinions are based on theoretical and practical experience,and the findings are desirable.2.On the basis of the average value of the importance of experts,AHP was used to determine the weight of the evaluation indexes of the physical restraint nursing quality of patients in the intensive care unit,and a logical consistency test was carried out on the index system.The single ranking and total ranking consistency ratios are both less than 0.1,and the consistency test results are acceptable;the weight value structure quality of the first-level indicators is 0.1667,the process quality is 0.6667,and the result quality is 0.1667.There are several factors associated with the quality of physical restraint,for example,training,period of restraint,nursing care,et al.Among the three-level indicators,The training of medical staff on the knowledge of physical restraint,standardized restraint stop record(stop basis,location,time),and physical restraint rate.ConclusionsCombining the Delphi method and analytic hierarchy ensures the scientificity and reliability of the quality assessment index system of ICU physical restraint.The index system is characterized with the clear hierarchy,strong,and can comprehensively measure the quality of physical restraint care of ICU patients.
