Objective:To investigate and understand the status of nurses' knowledge,attitude,and behavior associated of physical restraint in ICUs,using Delphi method to construct the training curriculum for physical restraint practice of ICU nurses,so as to provide the theoretical basis for the training of physical restraint,and provide a guideline on the clinical practice of physical restraint for ICU nurses,and to regulate the reasonable use of physical restraint in ICUs and improve the quality of nursing.Methods:1.Cross-sectional study: By using the continuous sampling method,110 nurses from 5 ICU departments of a third-grade class-A hospital in Shenyang were selected as the research objects.The general data questionnaire and the ICU nurses' knowledge,attitude and behavior of physical restraint questionnaire were used to investigate the status quo,so as to understand and analyze the current situation of ICU nurses' knowledge,attitude and behavior of physical restraint.SPSS 22.0 software was used for data analysis.Statistical methods included descriptive analysis,t test,one-way analysis of variance and Pearson correlation analysis.The test level was set as 0.05.2.Delphi expert consultation: Extensively searching and analyzing domestic and foreign literature,referring to the ICU physical restraint clinical practice guideline,and combining the results of the previous status survey,the initial content of the training curriculum for physical restraint practice was formed.Delphi expert consultation was conducted on the preliminary training course content.A total of 18 experts meeting the selection criteria were selected,and two rounds of consultation were conducted by paper questionnaires,e-mail and WeChat.Adjusted curriculum content according to the consultation result and the result of group discussion,and finally the content of the training curriculum was determined.SPSS 22.0 software was used for data analysis,and the authority and reliability of Delphi expert consultation results were expressed by such indicators as expert positive coefficient,expert authority degree,expert opinion concentration degree and expert opinion coordination degree.Results:1.Cross-sectional study:(1)1.The knowledge score of ICU nurses' physical restraint was(11.11±2.31),and the scoring rate was 69.43%,which was in the medium level.The attitude score was(33.33±3.71),and the scoring rate was 64.09%,which was in the medium level.The behavior score was(49.66±6.95),with a scoring rate of 82.77%.(2)Analysis of Influencing Factors showed that the scores of physical restraint behavior of ICU nurses at different ages were statistically significant(P<0.05),but there was no significant difference in scores of physical restraint knowledge and attitude(P>0.05);The scores of physical restraint knowledge and behavior with different working years in ICU were statistically significant(P<0.05),while scores of physical restraint attitude were not statistically significant(P>0.05).The scores of nurses' behavior of physical restraint in different ICU departments were statistically significant(P<0.05),while the scores of physical restraint knowledge and attitude were not statistically significant(P>0.05).(3)Pearson correlation analysis showed that ICU nurses' knowledge was positively correlated with attitude score(r=0.249,P<0.05).There was a positive correlation between attitude and behavior of physical restraint(r=0.239,P<0.05).2.Delphi expert consultation: During the two rounds of Delphi consultation,the expert positive coefficient was 100%,100%,the expert authority coefficient was 0.81,0.83,and the second round Kendall coordination coefficient was 0.280(P<0.01).In addition,according to the screening criteria of indicators and combined with expert advice,the indicators of each round were discussed,analyzed and revised in detail,and a scientific and reasonable training curriculum system of physical restraint practice for ICU nurses was finally determined,which was composed of 3 first-level indicators,14 second-level indicators and 45 third-level indicators.Conclusion:1.The knowledge of physical restraint of ICU nurses is at medium level,their attitude of physical restraint tends to be neutral,and their behavior of physical restraint needs to be further standardized.2.The training curriculum system of ICU nurses' physical restraint practice constructed in this study is found by Delphi method that it is scientific,reliable and consistent,which can better meet the actual needs of ICU nurses and can effectively guide teaching and clinical practice.
