Construction Of Physical Restraint Reduction Program For Adult Patients In Intensive Care Units

Research Purpose:To construct the physical restraint reduction program for adult patients in intensive care units suitable for China's national conditions,and clarify the the management,education and the implementation of specific processes of physical constraints,which can provide reference basis for clinical to develop physical restraint reduction project.Research Methods:1.Based on the system evaluation method of evidence-based medicine,the databases of Chinese and English at home and abroad and the related guide websites were searched.The related papers about reducing physical restraint use for adult patients in intensive care units were obtained and evaluated.The evidence was extracted,summarized and graded.Through the discussion of the research group,we drew up the first draft of the program.2.Using Delphi technique,two rounds of inquiry were carried out to 16 clinical experts in the critical care areas of the province.Through the reliability analysis of the results of expert inquiry,the final draft of the program was established.Research Results:1.A total of 29 literatures were included in the final study,including 3 clinical practice guidelines,3 clinical practice criteria,1 JBI evidence summary,7 systematic evaluations and reviews,and 15 original studies.The initial programme about the physical restraint reduction for adult patients in intensive care units covered five aspects: management of physical restraint,education and training,evaluation,alternative measures and implementation of physical restraint.2.In two rounds of inquiry,the effective response rates were both 100%.Authoritative coefficients of expert communications were 0.89 and 0.91 respectively.Coordination coefficients of expert communications ranged from 0.158 to 0.340(P<0.01).In the second round of correspondence,the coefficients of variation of indicators importance ranged from 0.00 to 0.19,and the coefficients of variation of indicators maneuverability ranged from 0.05 to 0.24.The finalized programme consisted of five dimensions and 40 specific indicators.Research Conclusion:This study based on Evidence-based and Delphi Technique constructed the physical restraintreduction program for adult patients in intensive care units,which has high levels of scientificity,reliability and rationality,and has certain theoretical and guiding significance to clinical nursing work.
