Construct The Participation Health Care Scheme Of Patients With Permanent Colostomy

ObjectivesThis study is based on the theoretical framework of patients participation in patients safety.Construct a set of complete,scientific,and systematic targeted and practical scheme of permanent colostomy patients participate in health care under the guidance of the concept of "patient participation in patient safety".The scheme provides a method and theoretical basis for patients to participate in self-health care,and provides a reference for clinical practice and continuous nursing practice.MethodWe systematically searched the relevant literature of patients with permanent colostomy participating in health care,analyzes the research content of the included literature by content analysis method,and extracts the contents and specific practices of patients of permanent colostomy participating in health care.Using phenomenological research methods,through purpose sampling and convenience sampling,9 permanent colostomy patients were selected for semi-structured interviews and in-depth personal interviews.The classic Colaizzi seven-step analysis method was used for data analysis.Based on the theoretical framework of patient participation in patient safety,combined with the results of literature analysis and qualitative research,established a preliminary scheme of permanent colostomy patients participate in health care in the "decisive involvement,caring involvement and complaining involvement" three participation strategies and 18 aspects.The Delphi expert consultation method was used to conduct two rounds of consultation with 32 experts,and the contents of the scheme were modified and improved according to the expert’s opinion,finally a participate in health care scheme of patients with permanent colostomy have been construct.ResultsA total of 19 articles were included,and 24 items including 13 aspects were extracted after content analysis.Through qualitative interviews with 9 patients,three types of participation and 18 types of participation were extractedIn this study,a total of 32 experts participated in the expert consultation.The analysis of the general information of the experts and the degree of authority of the experts showed that the experts consulted this time have good representative and authority.The recovery rate of the two rounds of expert consultations was 100%,and the authoritative coefficient of the experts was 0.84 respectively.After two rounds of expert consultations,the coordination coefficients of the importance and operability of the items were respectively 0.082 and 0.061.The final scheme includes 3 types of participation strategies,18 specific participation behaviors,69 basic contents,and each content gave a specific guidance planConclusions1.Through systematically searched and content analysis methods,this study found that there are many foreign literature related to the participating in health care of colostomy patients,mainly involving qualitative research and systematic reviews There is a certain evidence-based method basis and certain guidance.The domestic literature is mainly randomized controlled trials,the specific methods are different,and there is still a lack of a comprehensive and feasible scheme of permanent colostomy patients participate in health care for reference2.Through interviews on the participation experience of patients with permanent colostomy during hospitalization,three types of participation and 18 types of participation behaviors were analyzed.The main type of participation during the hospitalization was nursing participation.The content of appealing participation is relatively less,mainly for participating in questioning supervision.According to the main participation behaviors of colostomy patients during hospitalization,it can provide targeted guidance and promotion of patients participation behaviors,and provide a framework and content for the scheme of health care programs for colostomy patients.3.At present,there are no scheme of permanent colostomy patients participate in health care in China.This research method is rigorous.Based on the theoretical framework of patient participation in patient safety,combined with the results of literature analysis and qualitative research,a scheme of permanent colostomy patients participate in health care have been construct.The results of two rounds of Delphi expert consultations are reliable and credible.Experts are highly motivated and authoritative,and the importance of each items and the coordination coefficient of operability are good.It can provide reference and basis for the promote of patients’participate in self-care.
