Analysis On Present Situation Of Post-hospital Care Needs Scale In Patients With Colostomy And Validity Of The Research

Objective:Build demand scale for Post-hospital intestinal colostomy patients and test the reliability and validity.To provide effective assessment tools For nursing workers,and provide theoretical reference basis to carry out targeted nursing intervention.Methods:(1)A iteam pool was established:Through the investigation of relevant literature at home and abroad,Based on the Maslow's Need Hierarchy Theory,in combination with intestinal colostomy patients psychological scale,using qualitative research,a semistructured interview was conducted in 12 patients with colostomy A thorough understanding of colostomy patients Post-hospital care needs content(interview time as follows: September-October 2015),to analysis the interview recording data,the item pool of the scale for post-hospital care needs in patients with colostomy was constructed.(2)Pretesting scale formation:Picking up 9 medical and nursing experts relevant to colostomy from six the third-grade class-A hospitals in China.using Delphi method,after two rounds of expert enquiry for the items and content validity evaluation,remove ambiguous items,the useful items should be added,formed six dimensions Pretesting scale.The purposive sampling method is adopted in 20 patients with intestinal colostomy for small sample test(November 2015-January 2016)(3)Scale formation and inspection: in February to October 2016,through convenience sampling method,choosing 200 patients from the depatrment of Proctology of third-grade class-A hospitals of jilin province,and carries on the scale of clinical testing,after collecting the relative data,and make use of Criticial Ration?the correlation coefficient method and the internal consistency coefficient method?further concludes that the scale items should be added or not? and picking up the internal consistency reliability 1/2 reliability retest reliability content validity and structure validity to analysis each item.RESULTS:(1)Scale formation through the literature review?patients combined with the Delphi method was carried out on the nine experts,form the initial scale includes 24 items,after a preliminary survey of 20 patients after colostomy,Screening was carried out on the entry form six dimensions of 24 items after formal test scale.(2)The reliability of the scaleTotal scale of this research Cronbach a coefficient is 0.751,the dimension Cronbach a coefficient is 0.968-0.974,above the average of 0.7,shows good internal consistency reliability 0.794 scale of binary;Retest reliability of 0.827(3)The validity of the scaleContent validity of the scale overall S-CVI 0.968,24 items I-CVI value in 0.78-1.00.scale overall adequacy measure KMO value of 0.913,parameter values larger than 0.900,proved there is a common factor between variables;Bartlett the approximation of spherical test chi-square distribution is 4653.818,the degrees of freedom is 276,significant probability value P?0.01,reached significant level,illustrate scale project is suitable for Factor analysis,extracting principal component six factors six principal component accumulated to explain the variation degree was 91.753%,show that the scale has better validity Conclusion:1.This study completed the intestinal colostomy Post-hospital Care Needs scale2.The reliability of the iscale is ideal,scientific and accurate comprehensive reflect intestinal colostomy care needs of Post-hospital.Using this scale to evaluate care needs.
