Objective The application of collaborative nursing mode of patients underwent colostomy intervention effect of self-care ability.Methods Adopt the method of convenient sampling, the selection from January2013to October a first-class ternary hospitals in shandong province gastrointestinal surgery in the hospital with Miles operation (joint) colorectal cancer radical abdominal perineal treatment of96patients with low rectal cancer were investigated. The patients according to random method is divided into intervention group and control group each48people.Intervention group collaborative nursing mode, the control group routine nursing.In patients during hospitalization and after discharge,1month,3months, respectively, the application of self care ability scale (ESCA scale) and colostomy self-care ability questionnaire self-care ability, colostomy self-care ability, application of colostomy skin assessment tools-DET score evaluation colostomy skin recovery, and compare the two groups, explore the intervention effect of collaborative care model.Results1. Self-care ability of patients underwent colostomy1.1ESCA score:preoperative (114.45±26.43), postoperative7-8days(125.10±22.43), one month after discharge(32.23±19.26),3months after hospital discharge (135.78±19.04).1.2ESCA score level:at four time points lower level0points;Preoperative medium level38.54%, higher level accounted for61.46%;After7-8, medium level30.20%, higher level accounted for69.79%;After1month of medium level15.62%, higher level accounted for84.37%;After3months of medium level12.50%, higher level of87.50%.2. Of patients underwent colostomy colostomy self-care ability2.1After colostomy self-care ability score:7~8days (49.46±11.11), one month after discharge (61.57±7.09),3months after hospital discharge (63.73±6.86).2.2Colostomy self-care ability of three dimensions:7~8days, postoperative self-care intend to a higher level ratio is higher than low, nursing knowledge and nursing skills scored higher than low proportion of the level,1month,3months postoperatively, self care will, self-care knowledge, self-care skills were higher than low proportion of higher level.3. Intervention effect of collaborative care model3.1Two groups of patients with ESCA scores were increased.Different time points in the two groups ESCA score comparison:no difference (P>0.05), preoperative and postoperative7~8days,1month,3months after hospital discharge have difference (P<0.01).3.2Two groups of colostomy patients with self-care ability scores were increased. Differenttime points in the two groups of three dimensions:self will had no obvious difference (p>0.05);Self-care knowledge,7~8dayspostoperative1month after discharge,significant differences (p<0.05),3months after discharge without obvious difference(p>0.05);Self nursing skills were significant differences (p<0.05).3.3DET score comparison:two groups of patients discharged from hospital after1month,3months two time points, the intervention group had no skin erosion and tissue hyperplasia, in the color degree are better than the control group (P<0.05).3.4Two groups of ESCA score, colostomy patients with self-care ability score, self-care knowledge score, self-care skill score, DET score in the intervention group and control((P<0.05)) group with significant difference, and intend to score in no difference between the two groups.Different time points, ESCA, colostomy self nursing ability score, score will score, nursing knowledge score, self-care skill score, DET ratings have obvious difference((P<0.05)). Conclusion1. The model of collaborative nursing care of patients underwent colostomy is applied in intervention, self-care ability of high level and a growing proportion of patients with self-care ability enhances unceasingly.2. The model of collaborative nursing care of patients underwent colostomy is applied in the key of the intervention time is one month, can make the patients master self nursing knowledge and skills quickly and efficiently.3. Self care of patients underwent colostomy is enhanced by the sense of responsibility and willingness with the extension of the postoperative time.Hospital nurses, the patient and family interaction, the application of collaborative care model for health education, skills training, etc., can greatly improve the patients’ willingness to self care.4.The patient underwent colostomy improve self-care skills can effectively reduce the colostomy complications of the skin.
