Purpose : 1.To observe the clinical efficacy and safety of Yiqi Yangyin and Huatan Tongluo Decoction in the treatment of coronary heart disease with deficiency of both Qi and Yin and phlegm and blood stasis after percutaneous coronary intervention(PCI)。2.The effects of Yiqi Yangyin and Huatan Tongluo formula on the pathological changes of NO,ET-1,PGI2,AT-Ⅲ,Fib and myocytes in acute myocardial infarction(AMI)rats were observed.Methods:1.Clinical observation: from August 2017 to August 2019,60 hospitalized patients with angina pectoris diagnosed as coronary heart disease and PCI were selected to go to the Department of Cardiology of Anhui Hospital of traditional Chinese medicine.Randomly divided into control group and treatment group,there were 30 cases in each group,all were treated with western medicine,the treatment group was added with Yiqi Yangyin,Huatan Tongluo formula(granules)on the basis of Western medicine treatment,one dose per day,both groups were treated for 4 weeks.Improvement of TCM symptoms,BNP,LVEF,NO,ET-1,PGI2,AT-Ⅲ and Fib were observed before and after treatment.2.Animal experiment: 60 SD male rats were randomly divided into four groups: Yiqi Yangyin,Huatan Tongluo high dose group,Yiqi Yangyin,Huatan Tongluo low dose group,Tongxinluo group,model control group and sham operation group.After the model was established successfully,each group was given corresponding medicine or normal saline for 2 weeks.The expression of no,CK-MB,AST,c Tn I,LDH,ET-1,PGI2,AT-III and Fib were detected by nitrous acid reduction method,enzyme-linked immunosorbent assay(ELISA).The myocardial samples of rats in each group were made by HE staining after the experiment,and CK-MB,AST,c Tn I,LDH,NO,ET-1and PGI2 were observed after the treatment The expression level of PGI2,AT-Ⅲ,Fib and the pathological changes of myocardial cells in each group under light microscope.Results:Clinical observation:1.In the aspect of TCM symptoms,In the contrast group,the total effective rate is76.67%,96.67% in the treatment group,which was better than that in the control group(P < 0.05).2.For BNP and LVEF,the BNP and LVEF of the two groups decreased significantly after treatment,and LVEF increased significantly(P < 0.05);after treatment,the effect of the control group was lower than that of the treatment group(P < 0.05).3.For NO and AT-Ⅲ values,increased after treatment than before obviously(P<0.05),in terms of ET-1 and Fib,after treatment,the two groups were lower than before treatment,with a statistically significant difference(P<0.05);in terms of PGI2,after treatment,the two groups were not significantly higher,with no statistically significant difference(P>0.05).4.During the clinical observation,no obvious adverse drug reactions were found.Animal experiment:1.In terms of myocardial enzymes(CK-MB,AST,c Tn I,AST),no matter 24 hours or 14 days later,each treatment group was significantly higher than the sham operation group(P<0.05),in which the high-dose group of Yiqi Yangyin,Huatan Tongluo formula was close to the Tongxinluo group(P>0.05);compared with the model group,the myocardial enzymes content of the high-dose group and Tongxinluo group were lower(P<0.05);the low-dose group in CK-MB,l There was significant difference in DH level(P<0.05).2.In the aspect of ET-1,the content of ET-1 in each treatment group was lower than that in the operative group(P< 0.05),in which the high dose group of Yiqi Yangyin and Huatan Tongluo formula was close to that of Tongxinluo group(P>0.05),and these two groups are differences compared with the low dose group of Yiqi Yangyin and Huatan Tongluo formula(P<0.05).In the aspect of no,the level of no in the high dose group and Tongxinluo group was higher than that in the model group(P<0.05),and between groups have obviously distinction(P<0.05).3.In the aspect of AT-Ⅲ,after treatment,the levels of AT-Ⅲ and PGI2 in each treatment group were significantly higher than those in the model group(P<0.05),in which thehigh dose group of Yiqi Yangyin and Huatan Tongluo formula was close to that of Tongxinluo group(P>0.05),and between groups have obviously distinction compared with the low dose group of Yiqi Yangyin and Huatan Tongluo(P<0.05).In PGI2,there was no significant difference between the low-dose group and the high-dose group(P >0.05).Others are the same as AT-Ⅲ.In FIB,the FIB level of each treatment group was lower than that of the model group(P<0.05),and between the high dose group and Tongxinluo group are similar(P>0.05).4.At the end of the experiment,it was found that compared with the model group,the high dose group and Tongxinluo group could significantly improve the edema and necrosis of myocardial cells.Conclusion:1.In the therapeutic effect of traditional Chinese medicine,BNP,LVEF,NO,ET-1,PGI2,AT-Ⅲ,Fib,Yiqi Yangyin,Huatan Tongluo formula combined with western medicine were superior to the western medicine alone.2.The animal experiments of Yiqi Yangyin and Huatan Tongluo prescriptions showed that Yiqi Yangyin and Huatan Tongluo prescriptions could improve endothelial function and coagulation function,so as to reduce the levels of ET-1 and Fib,and increase the levels of NO,PGI2 and AT-Ⅲ in rats with myocardial infarction.In addition,prescription has existence in the dose and concentration.In a certain range,increasing the amount of Yiqi Yangyin,Huatan Tongluo formula can achieve the best therapeutic effect.3.No adverse reactions were observed in clinical and animal studies.
