The serious crowding in emergency department is the main problem managers facing now,and the reason is that the phenomenon is serious which the patients are blocked for the beds are unavailable at the department patients should be transferred to.The health care system is much too complex.There are interactions and strength differences between different departments.It should be given priority to optimize the source configuration from the perspective of the whole system to alleviate patients' blocking.This paper based on the characteristics of emergency system of our country,studies the arrival and blocking of patient mainly.Patients have heterogeneity so the decisions they make are different when facing the same waiting queue information which is used as the basis of patients arrival decision model to estimate the patients arrival to system.At the same time,in the process of studying patients blocking,this paper considers the fact that patients will accept the service of the subsequent department temporarily,quantifies the effect of blocking on service parameters of each stage more accurately,and proposes a new blocking probability heuristic which is verified by using simulation method.On the base of it,this paper establishes a beds configuration optimization model on the goal of minimum total blocking probability which is applied in the emergency system of a large-scale general hospital,and the result shows that the blocking phenomenon can be improved effectively by optimizing the beds configuration of each department,patients blocking probability is most sensitive to the change of the first stage system beds.
