Clinical Study On Improving The Adaptability Of The First Complete Denture In Elderly Patients Through Swallowing Function Rehabilitation Training

Objective : In elderly patients with low alveolar ridge,the effect of complete denture restoration is not good.The repair effect can be improved by techniques such as planting,vestibular groove deepening,magnetic attachment,etc.,but some elderly patients have economic,physical and other reasons.Advanced treatment techniques cannot be accepted,which requires doctors to help patients improve their adaptive ability to improve the restoration of dentures.This article uses oral rehabilitation training methods to exercise the lip muscle function as much as possible to enhance the oral and maxillary muscle coordination ability,to improve the adaptability of complete denture and shorten the adaptation period in the elderly patients with alveolar ridge.As soon as possible,patients can resume chewing and swallowing functions,thereby improving the quality of life and reducing the burden on the family.Methods :To Select the cases of complete denture restoration in elderly patients treated by the stomatology department of the Shanxian Center Hospital Shandong province from June2016 to February 2019.The patients had low mandibular alveolar Ridge,aged 65 to 74,with an average age of 70.There is no abnormality in the oral cavity of the patient and no neuropsychiatric disorders.They were randomly divided into two groups.All patients were examined by the same physician,molded with alginate imprints and red specimens,molded with super hard gypsum,transferred to the mechanic's room after using the lower third of the surface method to take vertical jaw position relationship and swallowing and biting legally to take the leveled jaw position relationship,and then made by the same technician using the same Bayer teeth and hot coagulated resin dental powder,Eligible patients: 47 cases.The oral rehabilitation training was carried out after the patients first wore complete dentures in the experimental group(rehabilitation training group)of 24 cases.Regular medical advice to properly wear dentures and timely follow-up,signing informed consent for oral rehabilitation training.The first were the indirect exercise training: lip locking exercises,mandibular exercise training,tongue training,cold stimulation.The direct exercise training were followed,train diet and gradual transition from water to general food.The maximumtraining time is 5 weeks.The control group(conventional treatment group)of 23 cases conventional tooth wear.The regular doctor orders to wear the denture correctly and return to the clinic in time.After 1 week and 5 weeks of wearing teeth in the two groups,the swallowing function,chewing efficiency,and satisfaction were evaluated according to the evaluation criteria,and the data were recorded using SpSS13.0 software,the results of the data were counted.The dysphagia,chewing efficiency and satisfaction statistics were analyzed for two groups of teeth in 1 and 5 weeks respectively.Results :The swallowing function of the experimental group was significantly higher than that of the control group at 1 week and 5 weeks(p<0.01 at 1 week,p<0.05 at 5 week).The satisfaction of fixation,chewing,language,and comfort in the experimental group was higher than that of the control group at 1 week(p<0.05),and there was no difference between appearance,solid satisfaction and control group(p >0.05);The satisfaction of fixed position and comfort at 5 weeks was higher than that of control group(p <0.05),and there was no difference between chewing,language,appearance,solid satisfaction and control group(p >0.05);The chewing efficiency of the experimental group was significantly higher than that of the control group at 1 week and 5 weeks(p<0.01 at 1 week,p<0.05 at 5 week).The results showed that the experimental group improved the swallowing function,mastication efficiency and satisfaction degree in the early stage.Conclusion: The elderly patients with alveolar ridge low level can improve the feeling and motor function of oral organs caused by oral rehabilitation training after wearing complete denture,establish normal oral movement modeand,as soon as possible improve the coordination function of facial muscle group and swallowing cluster,re-establish the reflector nerve pathway of swallowing,and obviously improve swallowing function,the satisfaction,chewing efficiency,and shorten the adaptation period of the elderly patients with alveolar ridge low level.
