Willingness To Engage In The Treatment-care Combination Nursing Homes Among Student Nurses In Sichuan Province And The Influence Factors

Objective:This study investigate the intern student nurses in Sichuan Province with secondary vocational,higher vocational,undergraduate and graduate degrees,aiming to:(1)know about the current status of willingness to engage in the treatment-care combination nursing homes among the intern student nurses;(2)explore the influence factors on willingness to engage in the treatment-care combination nursing homes among the intern student nurses;Methods :This study adopted the quantitative combined with qualitative survey design method:1.Quantitative research:Questionnaire investigation was adopted.Firstly,1respective medical institution was selected from 5 regions(namely,the northwest,western Panxi,south,northeast and the Chengdu plain of Sichuan Province),separately,by means of stratified cluster sampling.Afterwards,the intern student nurses were selected from these 5 institutions as the subjects of questionnaireinvestigation.The questionnaire was constituted by 3 parts,namely,the General Information Questionnaire,the Facts of Aging Quiz1(FAQ1),and the Kogan's Attitudes toward Old People Scale(KAOP),which was used as the research tool to quantitatively investigate the current status of employment intention to work in treatment-care combination nursing homes among the intern student nurses,as well as the influencing factors.Data were analyzed using the SPSS24.0 software.The general data and scale scores were statistically described by mean,standard deviation and percentage;single factor analysis was performed using chi-square test;and multivariate analysis was carried out through the ordinal multi-class logistic regression analysis.2.Qualitative research:The deep interview had adopted the outline formed based on expert consultation and pre-interview as the research tool.Sticking to the principles of information saturation and purposive sampling,intern student nurses had been selected from a certain general hospital of level II and a grade-A class-3 general hospital in Sichuan Province as the interviewees;the Colaizzi phenomenological seven-step analysis was adopted to sort out and analyze the collected data.Results:1.Quantitative research:(1)76.60% of the intern student nurses had taken the geriatric nursing courses in their schools;48.03% of them were taught by teachers from thegeriatric nursing clinic of hospital;the total school hours of geriatric nursing course was mainly<40(44.42%);classroom theoretical teaching was the major cultivation method(92.67%),only 45.43% of the intern student nurses had received novitiate or internship training,only 47.89% of them were trained in the nursing homes as the internship bases,and the duration of novitiate or internship was mainly <2 months(nearly 66.25%).(2)Only 30.22% of the intern student nurses had clear cognition of the treatment-care combination nursing homes;and the main way to know about the treatment-care combination nursing homes were the geriatric nursing course(61.79%).(3)Only 38.60% of the intern student nurses had clearly expressed they were willing to work the treatment-care combination nursing homes after graduation.The results of multivariate analysis suggested that the intern student nurses with better cognition of treatment-care combination nursing homes,higher FAQ1 scores,and higher KAOP scores had remarkably stronger willingness to work in the treatment-care combination nursing homes.(4)A majority of intern student nurses had preferred the public nursing homes(84.65%);full-timework was expected by most of them(56.15%),but part-time job would not be rejected by them(30.43%),and the expected monthly income of full-time job was mainly 5,000 ~ 7,000 RMB(36.50%),while that of part-time job was 2000~4000 RMB(60.10%);a majority of them were more likely to take the nursing management positions(57.52%).2.Qualitative research:A total of 24 intern student nurses with different education levels were included in the qualitative research for intensive interview.The data collected were analyzed with the Colaizzi's phenomenological method,and the results suggested that the factors affecting the employment intention of the student nurses to engage in treatment-care combination nursing homes included:Behavioral Attitude(attitudes towards the elderly,the cognition of treatment-care combination nursing homes,career hobbies,occupation appreciation,and occupation competence),Subjective Norm(salary,occupation risk,working environment,career prospects and social status),and external driving factors(employment competition,national policies,and the tradition norm of respect for the elderly).Conclusions:(1)The employment willingness to engage in the treatment-care combination nursing homes among the intern student nurses is markedly lower than that reported in other more well-developed domestic areas.(2)Quantitative research found the main factors affecting the employment willingness to engage in the treatment-care combination nursing homes among the intern student nurses include: the cognition degree of the treatment-care combination nursing homes,mastering level of geriatric knowledge,and the attitude towards the elderly.Qualitative research found that occupational interest,salary and employment competition were also factors.(3)According to the influencing factors affecting the employment willingness,the government,medical colleges and nursing institutions should take corresponding measures to improve the employment rate for treatment-care combination nursing homes among the intern student nurses.
