Construction Of Training Model For Gerontological Clinical Nurse Specialist Based On The Core Competencies

Objective To understand the relevant theories and contents of the training of gerontological clinical nurse specialist(GCNS)at home and abroad,to determine the theoretical basis and core competencies of the study;To understand the expectations and requirements of cultivating target groups,subject experts and patient groups for GCNS;To construct the core competence system of GCNS in accordance with the national conditions of China,and to formulate the training program.Methods 1.Through literature research method,we searched the domestic and foreign databases according to the retrieval strategy,and read the related literatures about the GCNS.2.Using the method of semi-structured interview and self-made interview outline to investigate the expectation and demand of nurses,subject experts and patients to the training of GCNS.3.The expert consultation questionnaire was drawn up with the expert meeting method,and the final training program was determined by Delphi expert consultation.Results 1.Through the literature review,the Hamric advanced nursing practice theory is basically established as the theoretical basis for the formation of the GCNS.GCNS' core competency framework included the clinical nursing practice ability,counseling ability,coordination ability,communication skills,leadership and management ability,scientific research ability,ethical practice and decision-making ability,and initially formed 57 second level entries;In the training content,the foreign GCNS training courses were generally divided into compulsory(such as research methods,health policy,leadership and management courses),advanced nursing practice courses(such as advanced healthassessment,pathophysiology,health promotion skills)and GCNS courses(such as the evaluation and rehabilitation of the elderly,the mental health of the elderly,etc.)2.Interview results showed that the clinical practice ability,communication and coordination ability,and education and guidance ability were the main concerns of the three parties in the theme of core competence,in the theme of training content extracted,disease management,clinical assessment,and psychological care were the major concern modules,training methods according to the interview results of experts,the results are as follows: the minimum degree of education for admission is a bachelor's degree,the minimum working number of years is 5 years,the training form is the union of hospital and university,learning form is full-time study,training teacher's working life is at least 10 year.3.Through the expert discussion,the GCNS training program expert consultation questionnaire including 26 first level entries and 176 second level entries was preliminarily developed.After two rounds of experts' consultation,the final training program for GCNS was established,which was devided into 4 modules such as training objectives,training content,training methods and training evaluation,including 19 first level entries and the 158 second level entries.The response rates of the two rounds of expert consultation were 100% and 88.9% respectively.The expert's personal authority coefficient was more than 0.80,the group authority coefficient was 0.89,which showed a high degree of authority.The P value of the Kendall coordination coefficient(W)test of the second-round's expert consultation was less than 0.05,which showed a higher expert coordination degree,these facts indicats that the consultation result has a certain reliability.Conclusion The expert consultation questionnaires formulated through the expert meeting method are clear.Delphi's expert advice is focused on the results.The research results are real and reliable.The core content-oriented training content is closely related to the training objectives,and the modular training positioning is clear and accurate.The training content is practical and feasible.The training methods and evaluations are in line with the current situation of domestic nursing development and the needs of nurses.It has certain practical significance and can provide a certain reference basis for the training of GCNS,but it still needs to be tested in practice.
