Objective: To observe the change of medication adherence, psychological state and the quality of life in the children with epilepsy after systematic health education, to study the influence of systematic health education on treatment adherence and the quality of life in epileptic children, and to analyze the relative factors, then to explore the corresponding countermeasures, and to promote the treatment adherence and quality of life in epileptic children.Methods: We used a prospective case- control study method, and selected 51 cases of pediatric epilepsy from specialist outpatient of the pediatric neurology during July 2015 to December 2015, and randomly divided into experimental group of 26 cases and control group in 25 cases.(1) Health education methods: experimental group used the systematic health education methods. The intervention method included giving out the epilepsy children's health education science manuals, recording seizure diary, holding a specific series of lectures, watching epilepsy science video, joining in chronic disease management platform of We Chat about children epilepsy, holding happy weekend activities, etc. Control group only gave regular daily care instructions by oral in the process of routine consulting in clinics.(2) The psychological condition investigation: we evaluated that using the screen for child anxiety related emotional disorders(SCARED) and depression self rating scale for children(DSRSC).(3) The investigation of epileptic children's quality of life: we used the quality of life inventory measurement models evaluated that.(4) Using the basic situation investigation questionnaire, medication adherence and epilepsy knowledge questionnaire, we analyzed the situation of medication adherence and epilepsy knowledge and the related factors about the quality of life. We assessed the children's psychological status and children's quality of life before and after the systematic health education 3 months.Results: 1. The influence of systematic health education on treatment adherence and epilepsy knowledge: the children's medication adherence and their parents' s correct basic knowledge of epileptic seizure respectively rose from 34.6% and 19.2% to 76.9% and 53.8% after health education in the experimental group(P < 0.05).2. The comparison the epileptic children's psychological status before and after the education: children with epilepsy after systematic education intervention whose SCARED and DSRSC scale total score, respectively(11.88±6.99) and(7.15±5.42), was significantly lower than that before education [respectively(20.62±9.04) and(9.92±3.91)], and the score on generalized anxiety, separation anxiety and social phobia were also obviously lower than that before health education(P < 0.05).3. The comparison the quality of life in children with epilepsy before and after the health education: the physiological function score in the quality of life in children after health education was higher than that before health education [(84.38±11.52) and(75.24±12.34), P < 0.05]. In the total score and other factors score of life quality between experimental group and control group before and after health education, there was no significant difference(P > 0.05).4. The analysis of the emotional and survival quality: the results showed that there were negatively correlated between the anxiety disorder of epilepsy children and the survival quality total score, each factor score(P < 0.05), and most intimately related to the anxiety disorder total score(r = 0.579). The depressed disorders had no significant correlation with quality of life in children with epilepsy(P > 0.05). 5. The study on the relative factors affecting epilepsy children's quality of life: comparing the patient's individual situation and disease, family, parents situation and other factors form the basic situation investigation questionnaire, the results showed factors below children grades, peer relationships, children's knowledge about epilepsy disease and the one-child had an obviously effect on epilepsy children's quality of life(P < 0.05).Conclusions: Systematic health education can raise the knowledge about epilepsy for the parents, to improve the treatment adherence in children with epilepsy. Systematic health education can help children with epilepsy to alleviate the anxiety and depression, relieve the generalized anxiety, separation anxiety and social phobia, and improve the quality of survival by improving the body physiological functions.
