Objective:Coronary atherosclerotic heart disease(coronary heart diseases, CHD) is a clinical common disease, frequently-occurring disease of cardiovascular system, first in the world disease death spectrum. In recent years, CHD as a typical psychosomatic disease, their personality trait and comorbid depression, anxiety is considered closely related with the occurrence and development of CHD. Based on this study by coronary angiography(coronary angiography, the CAG) confirmed the CHD patients with the method of questionnaire, the research of CHD patients with different personality traits, comorbid anxiety depression and personality traits influence on the degree of coronary artery lesions, to explore the role of psychological factors in the occurrence and development of coronary heart disease, so as to provide effective for CHD prevention, treatment and prognosis of the prevention and control measures.Methods:(1) Select 136 cases of coronary heart disease patients which were diagnosed by coronary angiography(coronary angiography, the CAG) under the age of 65 as the research object. All them hospitalized during 2013.5-2013.10 in the first affiliated hospital of dalian medical university of cardiology.(2) 136 patients were taken elbow venous blood on an empty stomach the next day morning, with enzymic method serum totalcholesterol(TC), triglyceride(TG), low density lipoprotein(LDL), high-density lipoprotein(HDL), apolipoprotein-a(apol-a), apolipoprotein-b(apol-b) and make registration.(3) All of them are investigated with general information, Hamilton anxiety scale(HAMA) and Hamilton depression scale(HAMD), D type personality scale(DS14), A type behavior scale(TABPQ), to evaluate the patient psychology and personality at the third day after coronary angiography.According to the result of A type behavior scale(TABPQ) score divided the patients into two groups of type A personalities group and non-A type personality, according to the type D personality scale(DS14) divided the patients into type D personality, non-D type personality groups.(4) All experimental data was analyzed by SPSS statistics 21.0 Chinese version, comparison of inter-group quantitative data used t-test and the one of inter-group qualitative data used chi-square test. Comparison of A type personalities group and non-A type personalities, D type personality and non-D type personality group in the general information, clinical information, anxiety and depression scale, the result of coronary angiography(Gensini score)(P < 0.05) as the difference was statistically significant. Spearman correlation analysis D type personality group of coronary artery lesion severity(Gensini score) and negative emotion(NA) and social inhibition(SI), D type personality score(NA+SI), Hamilton anxiety scale(HAMA) and Hamilton depression scale(HAMD) score correlation(P < 0.05) was statistically significan.Results:(1) About 58.09% patients are coronary heart disease with A type personality, About 47.06% patients are coronary heart disease with D type personality.(2) A type personality group and non A type personality, in the drinking, family history of coronary heart disease(CHD) difference was statistically significant. D type personality and non-D personality group in the family history of CHD, drinking, solitary difference was statistically significant.(3) The ratio of A type personality anxiety state of 48.10% and the ratio of depressive is 27.85%, non-A type personality group, the incidence of anxiety was 52.63%, the ratio of depression was 7.02%,the two groups were no statistically significant difference in anxiety, but depression is statistically significant difference. D type personality anxiety state ratio of 67.19%, the depressive state ratio is 28.13%, the non-D type personality group were 34.72%, 8.4%, the difference of two groups in the state of anxiety and depression status was statistically significant. D type personality group had a significantly higher anxiety and depression.(4) D type personality and the non-D type personality degree of two groups of coronary angiography of coronary lesions(Gensini score difference was statistically significant.(5) A type personality and non-A type personality group in the triglycerides(TG), the difference was statistically significant. D type personality and non-D type personality group in the serum total cholesterol(TC), low density lipoprotein(LDL), high-density lipoprotein(HDL), apolipoprotein-a(apol-a), apolipoprotein-b(apol-b) difference was statistically significant.(6) Type D personality group Gensini score and negative emotion(NA) and social inhibition(SI), type D personality score(NA + SI) positive correlation(Rs =0.403, 0.438, 0.487, P<0.01); Associated with HAMA scores, HAMD scores are positive correlation(Rs = 0.242, Rs = 0.319, P<0.01).Conclusion:(1) The proportion of A type personalities in hospitalized patients with coronary heart disease(CHD) is about 58.09%, D type personality of about 47.06%.(2) D type personality groups more prone to anxiety, depression and other negative emotions.(3) D type personality has influence to the crown severity coronary angiography, coronary angiography in the diagnosis of coronary artery lesion severity, the person that weigh, NA+SI score higher.(4) D type personality group TC、 LDL、 HDL、 apol-a、apol-b were significantly higher than that of the non-D type personality group,lipid levels is probably the mediation approach.(5) As A proportion of the D type personality in CHD is lower than the A type personality, but the effect on CHD may be more heavy, the rational use of psychological personality inventory, timely screening out the D type personality, when it is necessary to give psychological behavior intervention, or permisson for CHD patients with D type personality more benefit.
