Objective:To explore the correlation between D type personality and physio-psychological characteristics in elderly patients with coronary heart disease, and provide effective basis for prevention and treatment to improve the prognosis of coronary heart disease.Methods:200 patients with coronary heart disease were enrolled from March 2010 to March 2011 in department of cardiology, the first affiliated hospital of Kunming medical college. All of them are investigated with, the type D personality (DS14), Zung' Self-Rating Anxiety Scale (SAS), and Zung' Self-Rating Depression Scale (SDS) on the day before discharge. In the same time clinical results of assistant examination and general information were collected. All experimental data was analyzed by SPSS Statistics 17.0 statistical package. Measurement data was expressed as x±s and counting data was expressed as frequency. The comparison of inter-group quantitative data used t-test and the one of inter-group qualitative data used chi-square test. P<0.05 is considered statistically significant.The comparison of the multiple sample mean differences and multiple comparison of sample mean differences used one-way analysis of variance (one-way ANOVA). The correlation with the two dimensions--negative affectivity (NA) and social inhibition (SI), anxiety scale (Y), and depression severity index (z), used linear correlation analysis.Result:46% elderly patients are coronary heart disease with type D personality. Compared with the non-D type personality group,type D personality group has more white blood cell count (WBC), red blood cell count (RBC),2-hour postprandial blood glucose (PPBS), and left ventricular structure index(P<0.05), but less high density lipoprotein-cholesterol (HDL-c), prothrombin time (PT), international standardization ratio (INR), Index of left ventricular function(P<0.05). The rest of the clinical physiological indexes were no statistically significant differences. Compared the two groups, type D personality group with anxiety and depression state has significantly higher scores than the incidence of non-D type personality group (P <0.05). The two dimensions——negative affectivity (NA) and social inhibition (SI), anxiety scale (Y), and depression severity index (z), have significant correlation (P<0.05). According to different coronary angiography (CA) diagnosis, hospital diagnosis and whether or not using percutaneous coronary intervention (PCI), elderly patients with coronary artery disease are distributed into different subgroups. The groups have significant differences in anxiety scale (Y), depression severity index (z) and type D personality scale score (D) (P<0.05). Compared the levels of understanding of coronary heart disease and interventional therapy between type D personality group and non-D type personality group, P=0.005,0.008, type D personality group is higher.Conclusion:46% elderly patients are coronary heart disease with type D personality. Compared with the non-D type group, the D type group has higher smoking level and longer duration of hypertension. D type personality group and the non-D type group have significant differences in the clinical physiological indicators of left ventricular structure and function. The D type group is more likely to have anxiety, depression and other negative emotions than the non-D type group. The D type one with coronary heart disease which also combines with other cardiovascular diseases gets higher scores of the psychological indicators than other subgroups. And the more severe of the CA diagnosis of coronary artery disease, the higher score of psychological indicators. The psychological indicators of elderly patients with the D type personality after PCI were higher than those of non-surgery group. The personality factors influence the quality of life and prognosis of the elderly patients after PCI. So, active intervention to the physiological and psychological indicators of the patients may be helpful to improve the prognosis of patients with coronary heart disease.
