Application Of PDCA Circulation Method In The Operating Room Nursing Quality Management Research

1 Research backgroundIn recent years, along with the social development and technological progress, the pursuit of quality is developing continously. Because of concerning national economy and people’s livelihood, the quality of the product or service quality wins wide attention. To achieve sustainable development, any social organization providing products or services must first ensure the quality of the product or service. Therefore, as the core part of organization management, the importance of quality management is growing. And promoting quality management target, through the innovative quality planning, quality control, quality assurance and quality, has become a core strategy of many organizations.Hospital is the social organization providing medical and health services. Compared with other industries, because its service focuses on life and health, the hospital demands a higher requirement for quality management. According to the increasingly fierce competition in medical market, most domestic hospitals have equated the quality management to a hospital survival and development of lifeline, and executed the quality management as the core module of the whold hospital management system. Based on a completely setted-up quality management system, through constantly promoting the standardization, procedural, institutionalization and standardization implement the optimization of the quality management elements under certain conditions, and ensure to provide the highest quality of medical services.In the hospital quality management system, operation quality management is the key subsystem. Compared with outpatient and inpatient area, operating room demands a higher requirement of quality management. The main reasons are:fisrt, as the main place for various type of surgery, intervention examination, treatment and emergency rescue, the operating room work possesses a higher risk, complexity and variety; Second, based on the rapid development of current medical science and technology, minimally invasive surgery, intervention therapy, as well as endoscopic treatments are developing with each passing day. As the upstream and the key link of treatment process for surgical patient, the quality of the running management of the operating room, not only directly influence the patient’s life safety, but also affect the quality and the security of the entire surgical system output service. How to actively adapt to the changing of the technical environment, take the most advanced management concept from home and abroad, and build the operating room nursing quality management system with comprehensive standardization, strict scientificity, high efficiency, strong adjustment ability, and high security, is the ultimate goal of many modern operating room managers.2 PurposeBy introducing PDCA circulation method of TQM total quality management concept into the operating room nursing quality management, to strengthen the operating room nursing quality control, optimize medical cooperation, form the continuous improvement mechanism of operating room nursing quality, and finally reach the aim of strengthening medical security, avoiding errors, and guarantee a better quality of overall surgical service.Through the cross-sectional study about the operating room nurses in P phase of the PDCA cycle, investigate the knowledge reserve of professional theory in the operating room nurses, and the status of nursing technical operation level. Assessment the quality of the running patient medical records, and determine the problems that exist in the present situation. Based on the satisfaction survey from surgeon, to find out the main cause that affects medical cooperation. To investigate the related factors that influence the execution of hand hygiene in surgical medical personnel. Using PDCA circulation method, in view of the status quo, find defects, make plans, and carry out improvement and optimization.3 MethodsThis study combined the befoobjre-and-after self-control study with cross-sectional survey research. The research objects were the operating room nurses and doctors participate in the operation. The doctors working in hospital were chosen by systematic sampling method, and doctors whose job number was end with odd number participated in the survey. Research data came from the cross-sectional study before the implementation of PDCA circulation method in the operating room and the effect investigation after the implementation of PDCA circulation method.Collection and analysis data with EXCEL and SPSS software, describe the implementation of PDCA circulation method in the operating room, and its effect on the operating room nursing quality management.4 ResultCompared the qualified rate of nursing documents writing before and after the implementation of PDCA circulation method, it was found that the passing rate of nursing documents writing was 90.8% before the implementation of PDCA circulation method, and was up to 94.02% after the implementation, increased by 3.22%. The results showed that after the implementation of PDCA cycle, nursing documents writing qualified rate increase significantly (P< 0.05).Compared the passing rate of theoretical examination about specialized nursing knowledge before and after the implementation of PDCA cycle, the results show that after the implementation of the PDCA cycle the passing rate of theoretical examination about nursing knowledge was obviously improved (P< 0.01); the passing rate of nurses theory examination at different levels had increased significantly (P< 0.05 or P< 0.01), in which the theory test levels of the practice nurses, N2 nurses, and N3 nurses increased significantly (P< 0.01).5 ConclusionIn the operating room nursing quality management, implementing PDCA circulation method can effectively improve the writing quality of operating room nursing documents, the passing rate of theoretical knowledge and operation skills examination, the compliance of hand hygiene in medical staff, and the satisfaction from surgeons to the jobs of operating room nurses. At the same time, the method also effectively improves the continuous improvement consciousness and comprehensive ability of the operating room nurses. In general, the PDCA circulation method can significantly improve the quality of the operating room nursing management, execute the continuous virtuous cycle of operating room nursing quality improvement, and is the effective management method worthing spreading widely in the operating room nursing quality management to promote.
