Research On Humanistic Caring Ability Based On Confucianism

Objective1.To explore the internal relations between humanistic caring in nursing and Confucianism in depth,then create the nursing human caring theory model which correspond with Chinese culture background.2.And to build evaluation index system through a variety of methods on humanistic caring ability in nurses which can response the characteristic of Chinese culture and national conditions realistically and scientifically, to provide scientific and effective evaluation tools for training and assessment nurses.3.Then to provide scientific basis to education of humanistic quality in nursing through investigating the status quo of humanistic caring ability of nurses.Objectsl.Firstly, selecting2areas of13experts for depth interview,they come from medical humanities researchers of4colleges in Guizhou province,and there are experts in nursing education,clinical nursing, related management working in5hospitals.2.Selecting from clinical nursing, nursing education, nursing management, medical humanities education in4fields of30domestic and overseas experts for consulting.3.By sampling some clinical nurses who come from different second or third level hospital in Guizhou province (8hospitals), then investigate the situation of their humanistic caring ability.MethodsBased on the literature analysis, theoretical analysis, in-depth interviews, grounded theory to create the theory model of nursing human caring.To explore the construction of evaluation index system of humanistic caring ability on account of Confucianism, the evaluation index system contain theoretical basis and basic principles, and protocol the framework of index system. Applicated of Delphi expert consultation method on screening indicators. Used Analytic Hierarchy Process to determine the weight indicator system. Utilized coefficient’s a, the correlation coefficient index system such as reliability and validity testing. Applied questionnaire survey investigate the situation of humanistic caring ability of nurses in Guizhou province. Composesed all data by Excel2003and SPSS16.0statistical software.Results1.Constructed the theory model of nursing human care which contain "benevolence" as the spiritual core,"ritual" as the carrier of implementation,"faith" as the standard of behavior,"harmonious" as the value orientation; and formed8dimensionalities’ theory model of nursing human care which focused on cognitive competence, ability of human sympathy, ability of psychological support, ability of courtesy behavior, ability of etiquette behavior, ability of nursing cordially, ability of coordination with communication, ability of constructing harmonious.2.Developed "the evaluation index system of nursing humanistic care"s ability based on Confucianism", including4indexes of first-level,8indexes of second-level,59indexes of third-level, three rounds of the survey of experts assigned to the indexes of central tendency is increasingly obvious that reducing the coefficient of variation tends to coordinate the coefficient tends to increase,indicating that the views of experts on indicators of a gradual increase in the level of coordination,indicators reasonable screening.3.Set the weights at all levels value. Application-level analysis,the use of the importance of both high and low points difference Saaty scale to identify, build a comparison matrix adoption by determine the weight of root and the largest eigenvalue and consistency in the level of total test. The results showed that consistency at all levels to meet the requirements of indexes in order to work out the value of an index weight are:0.4410,0.1236,0.3118,0.1236. And use the principle of multiplication to calculate the probability of indicators at all levels of the portfolio weights,as detailed in Appendix6.4.Consulted expert group to choose a reasonable,high credibility of the findings. The basic situation of the group of experts to conduct a qualitative analysis,as well as the extent of the group of experts on quantitative analysis of the authority(the authority of the expert groups for the values0.86), results show that the consulted expert group has good representation,and authority levels. Consulting questionnaire recovery are as follows:93.8%,100.0%and100.0%, the expert on this study has a high degree of support and cooperative intentions. Experts" opinion have a preferable concentrated extent and the diversity is less. The second round and third round survey of experts’ coordination coefficients were0.358,0.335, what test show significant difference (P<0.01), indicating that the higher credibility and preferable coordination of expert advice, and the findings is reliable.5.Evaluation index system has good reliability and validity. The study evaluate internal consistency credibility by calculating Chronbach’s a, and evaluate the content validity by calculating r’s value of Pearson’s related coefficient. The Chronbach’s a of total credibility was0.963and internal consistency was very good. A of indexes of second-level and third-level were0.639~0.886, showing index system of internal consistency was preferable. The second and third-level indexes R’s value of were0.512~0.847, all of related coefficients more than0.5, degree of correlation was greater, reflecting indirectly the structure validity of index system was preferable.6. The average score of clinical nurses’ ability of humanistic caring, which come from8hospitals in Guizhou province, was8.88±1.05. The results of independent sample’s t-test indicating that there were no significant difference among rank of hospital,gender, the only child and the non-only child for the ability of humanistic care (P>0.05), but there were many significant effect among marital status,having children or not, teaching students or not, taking charge of head nurse or not (P<0.05). Paired comparison of variance analysis indicats that nurses’ education background were no significant effect for the ability of humanistic care (P>0.05), but there were a lot of significant effect among nurses" age, work seniority, positional titles (P<0.05). Pearson’s product moment correlation analysis indicating that there were positive correlation for the ability of humanistic care (P<0.05), such as nurses’ age, marital status, having children or not, work seniority, positional titles, teaching students or not, taking charge of head nurse or not. Moreover, nurses" age, work seniority and positional titles ranked top three. Conclusion1.According to domestic and foreign research, work in this paper explored the internal relations between humanistic caring in nursing and Confucianism, structured its theoretical model, enriched the connotation and theory system in nursing humanistic caring.2.Created the evaluation index system of humanistic caring ability, this paper is consistent with Chinese culture and national conditions, and it can be used to evaluate Chinese nurses’ humanistic care ability with ideal reliability and validity.3.There were significant differences among nurses with different age, work seniority, positional titles, marital status, having children or not, teaching students or not, taking charge of head nurse or not regarding their abilities of humanistic caring. The cultivation of nurses’ humanistic care ability should be paid more attention in every nursing manager of hospital, especially aiming at all kinds of factors which affect nurses’ humanistic care ability, carrying out further training hierarchically, improving the professional value and humanistic quality education for nurses. Moreover, building harmonious humanistic surroundings and work atmosphere to promote humanistic accomplishment and humanistic care ability of nurses constantly.
