ObjectiveThe main purpose of this study is to describe early changes in the quality of life and find the influence factors of the patients who received continuity of nursing care after the permanent intestinal stomas operation, in order to provide scientific evidence for nursing staff to adjust the targeted intervention programs, give reasonable continuous care, improve patients'quality of life in the greatest degree.Methods60 patients who were diagnosed with rectum cancer and were given operation of permanent intestinal stoma from December 2009 to October 2010 received continous care outside the hospital, including telephone follow-up once a week to understand the patients'diet, bowel, complications and psychological status; stoma clinic review once a month, review the stoma situation, ET help solve the problem; stoma association once a month, at the time of learning knowledge patients also built self-confidence. Patients filled registration card and basic information when they discharged from hospital.They were guided to fill scale of quality of life when they were reexamined in the clinic at the time of discharge,the first month of discharge and the third month of discharge respectively. SPSS15.0 software package was used for statistical analysis.Basic information, ostomy conditions and the scale of quality of life were described by frequency, percentage, mean and standard deviation. The differences among three stages used of one way ANOVA.Univariate analysis of quality of life, the significance of the differences between or among groups were tested by two independent sample T test,and one way ANOVA. Quality of life in multivariate analysis used of multiple stepwise regression analysis.ResultsIn continous care of discharge, the founction and the global quality of life of the permanent colostomy patients would be better and better,all the symptoms were increased in the first month after discharge but decreased in the third month after discharge. Multiple regression analysis of quality of life showed that the statistical significance factors in discharge were age and education. The statistically significance factors in the first month were education, stoma care providers, defecation, treatment, complications. The statistically significance factors in the third month were age, income, medical insurance,stoma care providers, treatment, complications.ConclusionContinous care of discharge improved the founction and the global quality of life of the permanent colostomy patients. In the future, decrease symptoms in the first month of discharge is the emphasis of continous care.The influence factors of quality of life were distinct in different stages. The factors in discharge were age and education. The factors in the first month were education, stoma care providers, defecation, treatment, complications. The factors in the third month were age, income, medical insurance,stoma care providers, treatment, complications.In the future clinical care and community care work, both stoma therapist and ordinary nurse in the discharge continous care should take targeted intensive care, deeper level of health education and effective nursing interventions according to the actual situation of the individual,so as to compensate the lack of community care and family care,to make patients accept colostomy in the shortest time,to improve quality of life, and to return to normal life and work as soon as possible.
