Objective1.Based on the"Ahmadi"continuation nursing model as the theoretical basis and guided by"Omaha problem system-based preventive colostomy patients care problems and symptoms/signs"as a guide to improve existing problems of preventive colostomy patients and improve patient self-care Protection ability.2.Through the formulation and implementation of a prolonged nursing intervention program for patients with preventive colostomy,the We Chat platform intervention is the main,supplemented by telephone and outpatient follow-up,to enhance the awareness and understanding of colostomy patients,and reduce the psychological burden,Improve the stoma behavior,reduce the occurrence of complications,close the stoma as scheduled,improve the convenience of medical treatment,reduce the cost of medical treatment,and improve the quality of life.3.By describing the changes in the quality of life of the stoma and the incidence of complications from 4 to 12 weeks from discharge to discharge,improve the patient's health behavior and provide a basis for continuing care.MethodsA total of 84 patients with preventive colostomy in gastrointestinal surgery at a top three hospital in Inner Mongolia were selected from January 2019 to February 2020.According to the random number table method,it was divided into 2 groups,the control group was given routine care,and the intervention group was given Ahmadi model to continue care.At discharge,4 weeks after discharge and 12 weeks after discharge,the patient's quality of life,ostomy complications were evaluated,and the outcome of the patient's ostomy closure was evaluated.ResultThere were 84 cases in this study,and the final effective sample size was 81 cases,including40 cases in the control group and 41 cases in the intervention group.The baselines are comparable.SPSS 23.0 was used for statistical analysis of the data,and the specific results are as follows:1.The results before the intervention showed that the baseline data of the two groups of patients was not statistically different(P>0.05).The results of repeated measurement analysis showed that the overall quality of life in the intervention group was higher than that in the control group(F=24.429,P<0.001).The overall quality of life was affected by time factors(F=79.194,P<0.001),and migrated with the time of observation There was a significant increase,and the overall quality of life in the intervention group increased over time compared with the control group(F=17.714,P<0.001).2.There was no statistical difference in the incidence of stoma-related complications between the two groups at discharge(X~2=0.408,P=0.523),4 weeks after discharge(X~2=4.601,P=0.032)and 12 weeks after discharge(X~2=4.516,P=0.034)were statistically different.3.Survival analysis showed that X~2=8.967,P=0.003,the difference was statistically significant,and the intervention group had better ostomy closure than the control group.ConclusionAhmadi Continuing Care provides a coherent and dynamic model of care through four phases of orientation,sensitivity,control,and evaluation.We Chat is used as the main carrier to provide patients with practical and effective stoma knowledge,to continuously track the stoma situation of patients;follow-up of stoma specialist clinics after discharge,to guide on-site stoma related issues,and to ensure a good nurse-patient relationship and continuous nursing development of.Therefore,continued care of Ahmadi can improve the quality of life of patients with prophylactic colostomy,reduce the incidence of complications,and improve the outcome of stoma closure.The research confirmed that this nursing mode is feasible for clinical promotion.
