The aging of China’s population is becoming more and more serious,and the problem of chronic diseases in the elderly is becoming more and more prominent.As the elderly population grows older and their physical functions decline,the number of people suffering from diseases is rising,and the health problems of the elderly group have become a focus of attention.The treatment period of chronic diseases is often long and tricky,which brings great negative effects to the health of the elderly,and a series of problems caused by chronic diseases become the biggest test for elderly patients with chronic diseases.On this basis,advocating the concept of health management for elderly patients with chronic diseases,awakening their awareness of self-management,and improving their ability to self-manage chronic diseases can help their physical and psychological health development.Therefore,it is imperative to give full play to the role of social work in the field of elderly services,to make scientific and effective interventions for chronic disease patients,and to explore a set of self-management methods applicable to elderly patients with chronic diseases.This study adopts questionnaire,interview and observation methods to analyze the current situation of self-management ability and needs of elderly patients with chronic diseases in Project X.Combining with social work concepts,the study adopts group work methods to provide scientific guidelines on the self-emotional management ability and self-behavior management ability of elderly patients with chronic diseases,and to evaluate the achievement of service effect,the improvement of self-management ability of chronic diseases,satisfaction survey and follow-up evaluation.The evaluation and analysis were conducted on the goal achievement of service effectiveness,the improvement of chronic disease self-management ability,satisfaction survey and follow-up evaluation,which provided a practical solution idea and suggestion to better meet the needs of elderly chronic disease patients’ self-management ability improvement.The results of the study showed that the group work was significantly superior to the intervention of the chronic disease self-management ability of the elderly,and the self-emotional management ability and self-behavior management ability of the service recipients were significantly improved.This chronic disease self-management capacity enhancement group for the elderly is a practical exploration process,and there are still many shortcomings that need to be explored and learned continuously.
