The Law of the People’s Republic of China on Basic Medical Care and Health Promotion,which came into effect on 1 June 2020,for the first time legally included hospice care as one of the elements of a comprehensive and full-cycle medical care for life service.Hospice care is mainly a service that provides whole-person care for the body,mind,society and spirit for patients at the end of their illness,helping them to pass away comfortably,peacefully and with dignity.In the context of our ageing society,the needs of the elderly are becoming more diversified.Hospice care,as a whole-person care service,has an important role to play in helping terminally ill elderly patients at the end of their lives.According to the requirements of the relevant documents,social workers are part of the hospice professional service team.However,there are currently few practical examples and studies of social work interventions in the field of hospice care in China,and there is also a lack of appropriate management systems.In order to explore and understand how social work can provide professional services in hospice care,the author participated in the "Rest Station" hospice care service project in City C Nursing Home as a social worker.In my practice,all vulnerable groups involved in the hospice service are treated as clients of the social worker,including the elderly,their carers and their families.In this paper,the author uses the observation method to understand the current situation of hospice services in G Nursing Home and its problems and to analyse the causes of the problems;the survey method is used to understand the knowledge of the nursing home staff about hospice care and to explore potential clients.Based on the actual situation,and guided by the concept of holistic care and social support theory,Maslow’s hierarchy of needs,the professional approach of social work is applied to case work: services to meet the physical,psychological,social and spiritual needs of the elderly and their families through case management contacts in organisational network work;group work through mutual assistance network strategies,diversified services for carers,emotional relief and stress relief services.Group Work Through the provision of specialised services to hospice clients,their carers and families,we assist them in solving their problems and meeting their physical,psychological,social and spiritual needs.The final result is a five-in-one model of social work intervention in hospice care for people in need,with caregivers,family members,older people and social workers,and a summary and reflection on the whole process,with recommendations for social work intervention in hospice care at both the micro and macro levels The following is a summary and reflection of the whole practice process.
