| According to the latest information released by the International Diabetes Federation(IDF),the total number of people with diabetes in China is now 140 million.Given that the number of people with type 1 diabetes is only a small percentage of the total,it is often assumed that children or adolescents do not develop diabetes.However,according to the latest information,there are 56,000 cases of type 1 diabetes in children and adolescents(0-19 years old)in China.Once a child or adolescent is diagnosed with type 1 diabetes,he or she must rely on insulin therapy for life.Not only do they need to rely on their parents or other family members to do a good job of "fine" blood glucose management every day,but they also need to regularly review their blood glucose,liver function,eye function and other related indicators every year.Therefore,families with type 1 diabetes need to make more efforts in disease management than other diabetes.For more than 90% of families with newly diagnosed type 1 diabetes,there is a lack of knowledge about disease management.After a six-month internship at the X Children’s Hospital,the author communicated with the medical and nursing staffs and institutional supervisors and decided to enter the Department of Genetics,Metabolism and Endocrinology to provide professional social work services to families of children with type 1 diabetes: on the one hand,she went into the inpatient environment of the children’s families,established five families of children with diabetes as the study subjects,and used social work interview techniques and semi-structured interview outlines to fully collect the basic information of the study subjects and to anticipate the study results.On the other hand,we adopted the case management model,formed a case management team,formulated action goals and plans for the families according to the case management service process,and implemented them,exploring how medical social workers could use resource assessment,coordination and integration of resources in the implementation of the case management model to help the families relieve tension and improve health management.families to reduce tension and improve health management.The intervention of professional services aims to help families of type 1 diabetes patients to relieve anxiety and tension,to understand the disease and its effects rationally,and to improve the health management awareness and management skills of families,which can then relieve psychological pressure and burden to a certain extent and provide guarantee for the health management of patients’ blood glucose.It was found that the implementation of the case management model can,on the one hand,assist clients to enhance their positive beliefs about the disease,expand their social support network,increase their health management knowledge,and maintain a long-term regular and effective disease health management status;on the other hand,the implementation of the case management model can play a certain advantage in intervening in the health management level of families with children with type 1diabetes,including promoting case services On the other hand,the implementation of the case management model can play a certain advantage in intervening in the health management level of families with children with type 1 diabetes,including promoting the development of one-to-one services,integrating multiple resources and improving the service standard of social workers.However,the case management model also has certain shortcomings and limitations,which include: the long-term effects of case management services need to be further evaluated,and the professionalism of case managers needs to be improved.Finally,the study proposes recommendations for the implementation of the case management model in three aspects: promoting the connection of the roles of case management teams,promoting the development of the case management model,and promoting the development of policies and providing comprehensive support. |
