According to the seventh national census of the National Bureau of Statistics,there are 260.4 million elderly people over 60 years old in China,accounting for 18.7% of the total population.More than 30% of them are suffering from diabetes,and more than9.5% of them are suffering from type II diabetes.Generally speaking,diabetes has a protracted course and cannot be cured,so it is important to improve the self-health management of elderly patients with diabetes.However,as patients are older and have developed some bad habits that affect their health,they lack the ability to manage their own health,so there is a great need for society and families to make more efforts to help patients improve their self-health management skills.At the same time the current service groups for health management of elderly diabetic patients in China are relatively solitary,and the participation of social forces is also insufficient.The problem of self-health management of elderly diabetic patients,from theoretical research to social practice,needs to be improved.With the continuous development of social work in China,the field and scope of social work are expanding,and social work has begun to intervene in the study of health management of geriatric patients with diabetes.Medical social work will play an important role in the treatment process of the disease with its professional and targeted characteristics,which are more able to develop services from physical,psychological and social levels for geriatric diabetes.This study was based on the medical social work perspective,participated in the internship and practice process,and at the same time combined with the results of combing through the relevant literature to finalize the specific steps of the study.1.Analysis of the problems of self-health management of elderly patients with diabetes in the community A of Chongqing,combined with the current situation of self-health management of elderly patients with diabetes.2.After attribution analysis of the problems under the perspective of cognitive-behavioral theory,the needs of elderly diabetic patients were explored,and the key contents of medical social work intervention were determined.3.Analyze the necessity and feasibility of medical social work intervention services through participatory observation,questionnaires and interviews.4.The intervention of medical social work services was carried out from several levels,such as patients themselves,families and doctors.On this basis,the effectiveness of medical social work intervention in the self-health management service of elderly diabetic patients was discussed,and countermeasure suggestions were put forward for different levels,hoping that this study can provide reference for future medical social work intervention in the self-health management of elderly diabetic patients.
