With the change of urban-rural integration and the change of residents’ lifestyle and diet structure,the incidence of diabetes in rural areas is increasing year by year.Elderly diabetic patients in rural areas lack attention to self-management of diabetes due to their low educational level,poor living habits and insufficient sources of health resources.Therefore,this group needs special attention.Based on this,the author first conducted a preliminary investigation on the elderly patients with type 2 diabetes in W community through interviews and scale measurement,and learned that the elderly patients with type 2 diabetes in this community lacked knowledge of health management and had misunderstandings.The consciousness of health management is not strong,and self-behavior management is neglected;Lack of emotional management skills and thin social support network.In view of the above problems,the author further analyzes and determines the needs of elderly diabetic patients.Then,under the guidance of cognitive behavior theory and social support theory,the group service plan is formulated.A group consisting of 8 elderly patients with type 2 diabetes mellitus was set up with the ultimate goal of improving self-health management ability.The group carried out seven group service activities in four dimensions: disease cognition,behavior management,emotional management and the construction of social support network for elderly diabetic patients.After the group activities,the author evaluates the effectiveness of group services through pre-and post-tests,interviews,observations and satisfaction surveys.After the evaluation,the author made a summary,and the evaluation results showed that the intervention of group work in the study of improving the self-health management ability of elderly patients with type 2 diabetes in rural areas had a certain effect,and the cognitive ability,self-behavior management ability and emotional management ability of group members were improved,and their social support network was expanded.However,there are some shortcomings in this study,and the author summarizes and reflects on the shortcomings,hoping to provide reference for the group practice research on improving the self-health management ability of elderly patients with type 2 diabetes in rural areas in the future.
